"[N]ot only may different parties define disease differently, but there is a crucial difference between having the authority to define disease and having the power to impose one's definition on someone else -- for example, physicians on patients, patients on family members, judges on physicians or hospitals, legislators on everyone else. Finally we must distinguish between possessing the expertise to deliver treatment and the means of doing so, from possessing the political power to impose treatment on those who do not want it, or to extract it from those who do not want to give it. These are simple distinctions. If we fail to make them, it is because we prefer to pursue hidden agendas of domination and submission concealed by a rhetoric of disease and treatment."
