Machine name: This is listed on the main page of your webcontrol
Reply Email: Where you want the form to be mailed to after submission
Domain name: Your domain name with no www in front
Form page: Name of your HTML form page, it should be in your main directory
Subject: This is the subject that will show in the email you recieve.
Filename to redirect: This is only filled in if you want to provide your own success page.
please enter just the file name and make sure its in your main www directory.
Display as secure: Click this to provide the form action for use with our SSL
