Most forms below are generic forms for any Scouter across the country. If it is specific to the Chicago Area Council it is listed as such. Your council may have its own local forms that must be used. Finally, some of these forms may have been superceded. If a form that you would like to see is not here please email us the information and we will attempt to find the missing form. Also if you have a new or updated form to add please send us the link and it will be added so all other Scouters have access to it. Unless noted otherwise all forms are in Adobe Acrobat Format.

Eagle Application Checklist This form is for the Chicago Area Council and is a checklist to make sure everything is correct on the Eagle application before it is turned in to the Council Service Center. It does not need to be filed, just used as a tool to make sure the application is complete. Silver Beaver This is the highest award that can be given at the Council level for volunteer Scouters.
