ELDERLY & DISABLED COMMUNITY-BASED OUTREACH DENTAL PROGRAM

The mission of our “Elderly & Disabled Community-Based Outreach Dental Program” is to provide logistical support to local dentists and create a “dental home” for the elderly and disabled who do not have access to dental care where they reside. ReachOut Healthcare America, via its affiliate Home Dental Group, has over 11 years of proven experience supporting local dentists who eliminate barriers to care for this underserved population.

Millions of seniors and disabled Americans live in skilled and assisted living centers and do not have ready access to dental care. This lack of care compromises their overall health and well being. ReachOut Healthcare America via a unique Case Management Center staffed with 70+ bi-lingual Case Managers provides support for the facility, residents and their family members. 

Coordinating the delivery of care is the major obstacle. We have overcome this barrier by developing a proprietary paperless, internet driven, patient record keeping system to ensure all residents are treated in a timely fashion and periodic basis. We coordinate between the facility, attending physician, nursing staff, family members and local dental providers from the community.

A local dental provider from the community visits residents every six months to provide comprehensive dental care using state of the art portable equipment. Many of the patients are treated in wheel chairs and or bedside. Emergent care is available upon demand. All patient records are maintained at the facility where they reside. Standard of care and age appropriate clinical services are provided to all patients every six months.
