Proof of Insurance Form - If you are involved in an accident and you had liability insurance at the time of the accident; this is the form for providing that information to the department. (PDF format, 13 KB)
No Court Action Statement - If your driver license has been suspended as the result of the Safety Responsibility Law, and you have not been sued after two years from the date of the accident, you can file this form to have your license reinstated. (PDF format, 15 KB)
General Release - If you have completely reimbursed the other party involved in the accident, this form releases you from any further liability. (PDF format, 15 KB)
Conditional Release - If you are making payments to someone for damages you caused as the result of a traffic accident, you can send in a conditional release to avoid your driver license from being suspended. (PDF format, 17 KB)
