Air Quality Permit Applications
The following applications/forms are available in MS Word format or in Acrobat PDF format.
- MS Word forms can be filled out, printed, and mailed and you can save the information for later use.
- Online Acrobat PDF forms can be filled out online, printed, and mailed, but the information can not be saved for later use.
- Downloadable Acrobat PDF forms can be printed, filled out by hand and mailed.

*In order to complete an online form you need to have Acrobat Reader version 5.05 or higher installed on your computer. You can check the version by starting Acrobat Reader, selecting "Help" from the menu bar and then selecting "About Acrobat Reader" -- your version of Acrobat Reader will be shown near the top of the opened window. If you do not have Acrobat Reader installed, you can download the free Acrobat Reader plug-in.
