Forms and Procedures required for Obtaining a Permit to Drill, Deepen, or Re-enter an Oil & Gas Well
The following forms/applications are available for either downloading or completion online, or both:
- Downloadable forms are available in a **PDF format and may be printed out and completed by hand.
- Online forms are available in a PDF format to allow you to complete the form online and then print and mail the form.

Included with each form is a link to the Administrative Rules of South Dakota (ARSD) Article 74:09 and 74:10 section that corresponds to that form.
**In order to complete an online PDF form you need to have Acrobat Reader version 5.05 or higher installed on your computer. You can check the version by starting Acrobat Reader, selecting "Help" from the menu bar and then selecting "About Acrobat Reader" -- your version of Acrobat Reader will be shown near the top of the opened window. If you do not have Acrobat Reader installed, you can download the free Acrobat Reader plug-in.

Plugging and Performance Bond (Form 3 - $5000 individual well, $20,000 multiple wells) Bonds need to acknowledge the principle and surety. A South Dakota licensed agent must countersign. A Certificate of Deposit is acceptable when made out to SD Dept. Env. & Nat. Resources AND Operator. |

Surface Restoration Bond (Form 10 - $2000 individual well, $10,000 multiple wells) File this bond when the landowner, or lessee, is not a party to the Oil and Gas Leasing Agreement. Execute separately from the Plugging and Performance Bond. |
