He is said to be at his last tether and not far from ending their engagement.

But now it has been reported that Miley Cyrus has made one last ditch effort to save her relationship with fiance Liam Hemsworth. 

According to a new report in In Touch magazine, the 20-year-old has enlisted the help of a 'spiritual' marriage counselor to help the pair work out their differences.

'She’s hoping to save her relationship by enlisting a therapist,' an insider told the magazine.

'She knows that Liam has one foot out the door and that she’ll lose him if she doesn’t act fast.'

Even though sources say 'they’re both not happy', Miley is said to be hoping that the guidance will change things around... and fast.

There has been friction and rumours of a split between the pair circling since February after Liam was spotted leaving an Oscars pre-party with January Jones.

But it seems that the damage had already been done.

'Miley and Liam fight constantly because he’s always questioning if she’s the same girl he fell in love with,' the source said.

'(He) doesn’t understand all of her new changes in life.'

The news comes as Miley was officially crowned No. 1 on Maxim's Hot 100 List on Thursday.

'I'm so happy to be #1 on Maxims HOT 100! #myfanskickass #dreamsdocometrue,' Miley tweeted following the announcement.

And reports of the counselling also come after it was claimed that Liam's brothers Luke Hemsworth, 31, and Thor star Chris Hemsworth, 29, staged an intervention in an effort to convince their youngest brother to split with the Hannah Montana star.

Last month, the two elder Hemsworth boys reportedly had a sit down with Liam to tell them how they really felt about the troubled relationship.

'They want him to end the romance for good,'  a source told Us Weekly.

And it seems that their advice did not fall on deaf ears, as not long after the alleged meeting reports emerged that Miley and Liam had pressed the pause button on their wedding plans.

While Miley has continued to insist that the pair have not split, Liam is said to be slowly distancing himself from his young fiancee.

'Liam's pulling away,' an insider added to the magazine.

Since reports of problems in their relationship, the couple have opted to attend several big events separately.

Most recently, Miley hit the red carpet solo at the Met Ball in New York on Monday night.

And even when they have both been in Los Angeles, the pair have also opted to go out without each other.

A prime example of this was on May 1 - Miley attended a bash at Beacher's Madhouse in Hollywood, while Liam opted to visit Chateau Marmont, the two venues just streets away from one another.

Recently Miley addressed rumours of a split between herself and Liam, calling them false, and saying they simply work too hard to spend as much time together as they would like to.

‘I’m not home with my boyfriend all the time. We work,’ she said. ‘Every week it’s “Are they broken up? Because we don’t see a photo of them.” I don’t have time to go to Starbucks with my boyfriend every morning. I wish I did, but I don’t.

'I’d rather chill at my house and be there for the time I actually get to spend with him. And then I’m at the studio all day. He gets up to work out at six and I come home at five from the studio.’

The star added that she has been prioritising her album over everything else recently – including wedding plans.

She said: ‘I put this record before everything, and I’ve never done that with anything. I’ve put too much into this record to put anything else in front of it. ‘

Reports of Liam's brothers's disapproval of his relationship with Miley come after older brother Luke recently praised Miley and said she was already part of their family.

He told Entertainment Weekly magazine: 'She's great. She loves our kids, and our kids absolutely love her. She won our hearts.

'I find her really interesting. I find her very articulate, and years in maturity above her age.

'But at the same time, she's very much like Liam.

'I think a lot of people don't realise that they are really, really in love. They actually are a perfect match in a lot of ways.'

MailOnline have reached out for a comment from Liam's spokesperson but are yet to hear a response.

Therapy at that stage would be the right comment. When the retrorespect of marriage and life time commitment pushes them to the point of needing counseling, they are kinda screwed already aren't they.i also find it fascinating that Liam's camp is sending Miley warnings through the media. Last break up was about Miley getting his message to Liam out, this time it's his family warning her. They both need to grow up and understand as immature as it may sound, the cliques are there for a reason. Birds of a feather indeed

Those Aussie boys like their "Sheilas" simple, loving and wholesome. Aussie men are very protective and loving. Cyrus is a product of a broken country.
