Toni Braxton takes the stage in Chicago as she's released from hospital after latest Lupus flare-up

Toni Braxton was released from a Cleveland hospital Sunday in the wake of her second debilitating bout with lupus this month.

Braxton headed straight to her show in Chicago following her release from the hospital, after the ongoing illness caused her to postpone a gig slated for Cleveland on Saturday.

The Grammy-winner, 49, took to her Twitter page to update her fans on the latest in her grueling health battle.

'Hey guys, thanks for all your well wishes and understanding,'' she wrote. 'I am out of the hospital, and en route to Chicago! I can not wait to see you.'

Things went well for the performer in the Windy City, as the You Mean The World To Me singer posted a clip of her performance to her Instagram page declaring that 'Chicago was amazing.'

The Another Sad Love Song singer added, 'I cant wait to see you all on the road! Performing is what I love to do most, the connection my fans & I have is undeniable!'

After her show in Kansas City on Friday, the Un-break My Heart singer's health took a downturn, causing her to delay her concert as she checked into a Cleveland medical facility to tend to her flared-up lupus symptoms.

Braxton's team issued a tweet on the rescheduling, telling the singer's fans that while 'lupus needs to be constantly monitored ... this minor setback should not affect the rest of' her The Hits Tour, which next rolls into Tupelo, Mississippi's BancorpSouth Arena Thursday with another show slated for Horseshoe Casino's Bluesville in Robinsonville on Friday.

Earlier this month, the Breathe Again vocalist spent 'a few days' under medical supervision at a hospital in Los Angeles, according to an October 3 statement from her rep, who noted that Braxton 'was not in serious condition' as result of the flare-up.

The He Wasn't Man Enough crooner, who's romantically involved with rapper/producer Birdman, told The Huffington Post last year that 'the toughest part' of her battle with lupus was when she had to perform. She said that she's asked herself, 'How did I get through that?' after some of the more challenging outings.

Braxton added that she had a secret weapon in getting through the strenuous shows: her fans, who fill the arenas with their singing when she's not at 100 percent.

'When it's like that, it's a lot of, "You sing!" to the audience, you know,' she said. 'They kind of help me out a little bit without realizing it.'
