Medicaid is a health insurance program for individuals and families who cannot afford health care costs. Medicaid serves low-income parents, children, seniors and people with disabilities.

Apply online for Medicaid


ePASS is a secure tool that allows you to submit an application online with easy-to-use instructions. You may also apply at the Department of Social Services (DSS) in the county where you live, or you may print and mail completed Medicaid and Health Choice Applications to your local DSS office. You also may request that an application be mailed to you. If you are disabled, a county representative may be able to make a home visit to assist you with the application process.
