".   .   .   I was just looking at your website and saw the F-4 from Idaho National Guard that had a bird strike on 8 Apr 1982. It states that Englebreit and his back seater ejected. I was working in the fire department at Mtn Home AFB that day and was on that IFE. It is true that the back seater landed the aircraft and took the cable. Neither individual ejected as a result of the bird strike that day. I helped put the pilot in the ambulance after we egressed him. Just wanted to clarify that no ejections took place on that incident as the data for that incident seems to indicate. Thanks.

A A flight of two RF-4Cs doing an experimental SLAR mission over the Baltic Sea in an area we don't normally fly in.  The backseater in the mishap aircraft was 1st Lt Tim Brennan.    Bob Trombly was the lead WSO and was considered the sharpest Instructor WSO in the squadron.  Unfortunately an old fashioned cross check was never done on his calculations.  Bob's calculations were correct, but for the wrong scale map and so his fuel calculations were off by a factor of 50%.  Both aircraft should have diverted but ego got in the way.  The lead aircraft landed safely at Karup on vapors with one engine flaming out on taxi.  Hermandorfer's aircraft ran out of gas a mile short of the runway.

17 August 1984: F-15B, 75-0087, of the 1st TFTS, 325th TTW, USAF, collided with F-4E 68-0535 of the 526 TFS, 86 TFW "RS", and crashed into the Gulf of Mexico. One dead - the pilot of the F-4E - two survivors (the pilot of the F-15B and the WSO of the F-4E). The F-15B was being flown solo (no-one in the back seat). According to an eyewitness report: 

"I was with that deployment when it happened. It was a head-on engagement with a little offset between two F-4s and one F-15. As the aircraft approached 3-9, the pilot called a break into the F-15. Someone misjudged trajectories. The two aircraft hit wingtip to wingtip. The backseater waited to see if the pilot could recover the aircraft, but there was no apparent attempt to recover as the plane did horizontal 360s in the air. 

The GIB initiated the ejection, suffering minor flailing injuries and minor injuries to his face from his mask being ripped off by blast forces from the wind. He and the F-15 driver were picked up fairly quickly, but neither had seen the pilot of the F-4. 

His body was recovered early the next morning by a fisherman who saw his chute floating in the ocean. The seat was still 
attached to his harness, but his rubber duckies had been deployed, and he was tangled in the shroud lines of the chute. He was found to have a contusion to the side of his head. It was believed that he struck his head on the canopy when the collision caused the aircraft to spin violently, knocking him unconscious. They felt that sometime during his descent, he 
became conscious enough to deploy his floatation devices, but was pulled underwater by the seat and drowned".

  I was on the mishap board for the 6 Nov 1985 crash at Zweibrucken.  I was the recorder back before computers were common place.  I edited the report and with the assistance of a secretary using a word processor we put it together.  It was again pilot error.  The pilot was Capt John Griffen who was new to the RF-4.  He had a lot of time in Air Training Command and his back seater was Capt Cookie Welton an Instructor WSO.  In the RF-4C during high AOA maneuvering you cannot use ailerons (this is only true in the C model and earlier models, later models had slats), you need rudder to roll.  John knew this, but in a defensive maneuvering fight against an F-5 agressor he reverted back to ATC habits and used aileron.  This resulted in a snap roll in the opposite direction as the aircraft went out of control.  He executed the proper bold face procedures to recover and did recover, but subsequently put the aircraft twice in a stalled condition.  The problem, not a memory item, he forgot to advance the power back.  They both stayed with the aircraft too long as the bold face says "out of control below 10,000 ft eject."  They had only a few swings in their chutes before they hit the ground.  The aircraft recovered out of the stall on it's own and hit the ground at 425 kts 40 degrees nose low impacting the side of a hill in a wooded area.

I'm glad I stumbled onto your site.  I wanted to comment on 74-1056 that crashed at Herforst, Germany, on 26 Dec 1986.  The aircraft caught fire right after takeoff, and then Spicer and Wiggam punched out over the Spangdahlem Air Base Golf Course.  I was a J-79 engine mechanic working at the Spangdahlem test cell and the pilot and WSO landed right beside our facility.  On that day I just happened to be on leave in the states, but my co-workers witnessed the ejection (in fact, they found the pilot's checklists and other items strewn all over the golf course).  As I recall, a weekend duty crew had installed 2 each 370 gallon wing tanks on 74-1056.  When you fill the tanks on the ground, (I believe the procedure is called "over the wing" refueling) the fuel cap must be installed with the lock lever positioned so that aerodynamic forces won't "flip" the lock lever open during flight.  As fate would have it, the crew installed the cap backwards, and when the plane was barely airborne (and still in afterburner!) the lock lever flipped up, the cap blew off, and fuel began to "vacuum" from the tank.  The fuel subsequently blew aft and made contact with the afterburner flame, and one side of 74-1056 became engulfed in flames.  The Spangdahlem tower informed  Spicer and Wiggam that their aircraft was on fire, at which point they punched out.  The official accident report stated that the unmanned aircraft flew for another 40+ seconds and then plowed into a cemetery in the town of Herforst.  Luckily, the crew was OK and nobody on the ground was injured.  After, my co-workers and I also learned that members of the weekend duty crew that had installed the tanks each had some sort of disciplinary actions/discharges pending and thus should have been de-certified on the procedure.  Of course, the individual who signed the exceptional release also could have caught the error, as well as the crew chief, and the aircrew during walk-around.  A comedy of errors ending in a destroyed Phantom.  I know this is long-winded, but your website brought back a ton of memories of my days at "Spang-Quentin."  Thanks.  CMSgt, USAF (retired

The details are as follows, RF-4C 72-0146 of the 10 TRW at RAF Alconbury was performing several fly-bys at RAF Brawdy's airshow on the 24th July 1986 when it crashed into the sea with the sad loss of both crew. It appears the back-seater managed to eject before the aircraft impacted the sea, but was possibly outside of the seats envelope and did not survive. The pilot also appears to have initiated ejection but, too late and he also did not survive. The pilot was Capt Mark Makowski and the WSO was 2nd Lt Dewayne Danielson. It was reported that during their display that a panel was seen to fall from the aircraft, this was later recovered and said to be an engine panel, wether this had any bearing on the accident is unknown.

I was the pilot of aircraft 68-0448 which took off  from Lambert Field, St Louis, Missouri (Unit was 110 TFS part of the 131st  TFW all based at Lambert in STL) The aircraft crashed at 1315 on the grounds  of Ft Leonard Wood, Missouri, about 2 miles north of the Cannon gunnery  range. Cause of the crash was the internal failure/separation of the right  engine deflecting hot air to the spinal fuel bladders causing them to  rupture. Aircraft impacted in a wooded area and the only injury was a  compression neck sprain from the rear seat WSO, Maj Don Boatwright.

  They had just departed Bergstrom after their third or fourth touch & go when the #1 engine lost a compressor blade which puncture one of the fuel cells sending raw fuel into the engine bay fueling a massive fire.  The pilot, Col Grimsley, turned the aircraft away from Austin towards an open area before he and Capt Ganiko safely ejected.  Here is a link to an AP archive story on the mishap http://www.apnewsarchive.com/1988/Air-Force-Plane-Crashes-in-Austin/id-6061198de0640982dad1911a2c0b8835 .  I was stationed at Bergstrom from Oct 1986 to Jan 1990 and remember this one well.  I believe Col Grimsley was the deputy commander of the 67th TRW at the time.

 AUSTIN, Texas (AP) _ An Air Force jet crashed into a warehouse complex today, but both crew members ejected and parachuted to the ground, fire and Air Force officials said.

 

Airman Kim Lowery of Bergstrom AFB in Austin said the crew members from the RF-4C Phantom II jet were being examined by Air Force medical officers at Bergstrom. There was no immediate word on their conditions.

 

Lt. Larry Morris, Austin Fire Department spokesman, said there were no immediate reports of injuries on the ground.

 

Fire department officials said the blaze caused by the crash was brought under control about an hour after the accident.

 

The jet was returning from a low-level training mission when the crash occurred about 12:15 p.m., Ms. Lowery said.

 

The crew members were identified as Col. Roger L. Grimsley, 47, from Leesburg, Ohio, the pilot. Capt. Terence C. Ganiko, 30, of Honolulu, Hawaii, was the weapon systems officer.

 

The RF-4C is the reconnaissance version of the F-4 fighter-bomber.

 

Air Force officials said a special board of officers would investigate.

SAN BERNARDINO — The California Air National Guard member killed Thursday when a jet fighter crashed into her military pickup truck at nearby March Air Force Base has been identified as Sgt. Sheila R. Willits.

Meanwhile, an Air Force investigative board has begun its inquiry into the crash and will examine whether the pickup--from which Willits and two other guard members were monitoring the plane's takeoff--was parked in violation of guidelines governing how close vehicles may be to a runway in use.

Willits, 22, of Rancho California, was part of a ground crew observing the departure of the F-4E Phantom fighter. She was pregnant and is survived by her husband, Calvin Willits, a Marine stationed at Camp Pendleton.

The truck's other two passengers were injured and remained hospitalized Friday. First Lt. Mark Gabel, 29, of La Mesa, was listed in critical condition, while Sr. Master Sgt. Robert M. Smith, 45, of Corona, was in fair condition.

The plane's pilot was identified as Lt. Col. Donald E. Worden, 46, of Moreno Valley. Worden and the aircraft's weapons systems officer, Maj. William C. Gallegos, 38, of Carlsbad, ejected and suffered moderate injuries.

Maj. Michael Ritz, a spokesman for the Air National Guard, said the plane was in the process of taking off at 1 p.m. Thursday when it suddenly plummeted to the ground, struck the pickup and burst into a fiery ball. The plane was armed with 20-millimeter cannon shells and practice bombs and was headed out for a routine training flight over the high desert.

Ritz said it is "common and accepted practice" for an "authorized observation vehicle" to park beside the runway and monitor the takeoff of tactical fighter aircraft.

"They observe the standards and practices of the pilots and just watch how the aircraft performs," Ritz said. "It was not at all unusual" for the vehicle to be in the vicinity, he said.

Ritz said the truck was parked in a grassy area beside the runway, but exactly how close to the runway he could not say. Although there are Air Force and Air National Guard guidelines governing where vehicles may park during takeoffs, Ritz said details of those guidelines were unavailable Friday.

Whether the truck was too close to the runway is "certainly something that will be part of the investigation, and I can't speculate on it," Ritz said.

All of those involved in the accident were members of the Air National Guard's 163rd Tactical Fighter Group, which is stationed at March Air Force Base. The last fatal crash involving a member of the group occurred March 21, 1987, when entertainer Dean Martin's son, Dean Paul Martin, crashed his F-4C into the side of the San Gorgonio Mountains.
