Averil Hart (right and left, in hospital with her father Nic), 19, from Sudbury in Suffolk, was let down by every part of the organisation that should have cared for her, the Parliamentary and Health Service Ombudsman says. Yet the four NHS bodies refused to take responsibility for the tragedy and instead were ‘defensive and protective of themselves’, the report says. She was found collapsed in her university room after losing 2st – nearly a third of her body weight – in less than three months. Doctors had not properly tracked her weight or her mental health, and after she was rushed to A&E;, two hospitals made basic blunders that accelerated her death.

Hit and run driver 'flees the country' after a policeman 'refused to chase him across a field because he didn't want to get MUDDY'
Latvian suspect Marcis Blahins (right) is now believed to have fled the country following the incident near Newmarket, Suffolk, on Saturday night. Aaron Tokley (left), 29, whose Mercedes was written off in the crash (inset), was stunned by the officer’s response and accused police of failing in their duties. ‘I couldn’t believe it. They were more concerned with their fear of the dark and getting their uniforms muddy,’ he told The Sun. ‘All they needed was a torch and some guts.’ He says Suffolk police missed vital evidence which could have resulted in an arrest.
Take-off for Brexit? Plane on standby to whisk bleary eyed May to Brussels to sign Brexit deal after desperate night-long negotiations – but Irish border issue could still scupper agreement
Theresa May is said to be poised to fly to Brussels first thing this morning to finalise a deal with the EU that will unlock talks on trade after frantic night-long negotiations. After days of stalemate, British and European officials said they were ‘within touching distance’ of hammering out a 'divorce agreement' that satisfies the EU, Ireland and Mrs May’s governing partners in the DUP. It could see the PM head to Brussels as early as today if she wins consensus on a plan for the Irish border. An earlier outline of a deal on the key divorce issues was torpedoed by the DUP on Monday. The party objected to plans for ‘regulatory alignment’ between Northern Ireland and the Republic to maintain a soft border, arguing it would amount to the drawing of a new frontier with the UK mainland. European Council president Donald Tusk (right) last night announced he would make a statement on Brexit in Brussels this morning, fuelling speculation a deal was close.
The street where broadband is so slow it can take 21 HOURS to download a film: Staggering differences of Britain's 'Postcode Lottery' internet are revealed
Residents in Thorpe Lane in Suffolk’s Trimley St Martin hold the dubious honour of having the country’s slowest internet. They can’t use video-streaming services such as Skype in high-definition, and even checking Facebook can be frustrating. Their broadband speeds average just 0.68 megabits per second (mbps) – 53 times slower than for the average household and an incredible 260 times slower than the fastest speeds. Residents in Benford Avenue, Motherwell, North Lanarkshire, however, enjoy speeds of 177.1mbps, meaning they can download a movie in under five minutes.

Their intention was to use the microwave as a mould, and by the time emergency services were at 1.49pm on Wednesday to the garage of a house in Fordhouses, Wolverhampton, the group had already been trying to free him for 90 minutes. The friends had managed to feed an air tube into the man's head to help him breathe.

The mother, from Derby, was stunned when her pleas for help were ignored as she cradled her little boy in her arms and ended up running to find a doctor who resuscitated her son. But she ignored the order to wait in line and ran to find a doctor who resuscitated little Tommy-Lee (left) and put him on a life support machine. The mother-of-five (pictured right with her partner Jamie Brewin, Arlow-Jack (Tommy-Lee's twin and other children Layton-McKenzie Dexter-James, and Gracie-Mae) said her baby 'could have died' if she had not acted quickly.

Child minder, 30, feared she would be ‘disfigured’ for life after the lid of her thermal mug ‘collapsed’ and doused in SCALDING latte leaving her covered in 'excruciating' red welts and blisters

Abigail Jones (pictured right) was left screaming in 'excruciating' pain earlier this year when the instant latte she made with boiling water hit her skin. Red welts and blisters covered her chin (left), lower jawline, neck, chest and six inches of her inner arm (inset) after the accident in February. Driver Alfred Dorris, 43, dozed off for up to a 'few minutes', woke just before it flew off the track and was so disorientated he thought he was travelling in the other direction, an investigation has found. The seven commuters who died when the vehicle toppled over on November 9 last year (top left) were all thrown out and crushed by its sliding carriages. Marilyn Logan, whose husband Philip died in the crash, said today he would have been alive now if the windows had been laminated to stop people being thrown from the tram. It was travelling at 46mph as it entered a bend at Sandilands Junction where trains should not go any faster than the 13mph limit. New images show the devastated tram on its side near Croydon and inside its overturned carriages (main picture) with shattered windows, bent handrails (bottom left) and buckled doors. Survivors described the trauma of being stuck with no emergency lighting and desperately called 999 because they had no obvious escape.

Wish you were there? Postcards from 100 years ago reveal a snapshot of life in rural England when few cars clogged the roads and getting to work or school meant hopping on a bike or riding a horse-and-cart
An incredible collection of postcards depicting rural scenes from 100 years ago has been unearthed to give a fascinating snapshot of a more innocent time. The postcards show the quiet life in villages in south-west England in the early 20th century. At a time when the automobile was in still in its infancy, it is no surprise to see many of the streets virtually empty (top left) in many of the pictures. One image shows the schooldchildren of Curry Rivel, a tiny parish outside of Somerset, posing for a picture before lessons (bottom left). Although the horse and cart and the bicycle was the transport of choice for locals, one picture does show a motorised charabanc piled high with smartly-dressed men and women clearly off on a day out (top right).

Red-hot shots! Photographers risk their lives perching on the edge of a huge Hawaii VOLCANO to get the perfect shot
The two snappers were spotted balancing on the precipice of Mount Kilauea, on the Big island of Hawaii, while lava of more than 2,000 degrees Fahrenheit ran like rivers of fire beneath them. The daring duo, who ventured far away from the public viewing area, were spotted by photographer, Joseph Anthony, who saw them from the base of the volcano - the lava ocean entry point - during a boat trip. Kilauea Volcano is on the US Geological Survey's 'Watch list' and continues to erupt at its summit and from the Pu'u O'o vent on its East Rift Zone. The lava does not, however, pose a risk to nearby communities.
