Out of State Request for Driver's License Packet (secure)
(non-SSL). If you are temporarily
living outside Virginia, you may renew your
driver's license, obtain a military extension
or obtain a replacement driver's license by
mail.
Request an Administrative Hearing (secure)
(non-SSL) regarding an insurance-related Order of Suspension. If you have been issued an Order because you did not have liability insurance on your vehicle, you can use this form to request an administrative hearing.

Reader to view and print.
PDF forms can be used in two ways. You may either:
- print the form and then write in your information, or
- you may type your information into the fields provided on the form
and then print. Each form contains fields for you to type in your
information and then print the form. After selecting the form, click
