Aquatic
Farm Application and Instructions — Link to a

The Aquatic Farm application packet is designed to help you obtain
the authorizations routinely required by the State of Alaska Departments of
Natural Resources (DNR), Fish and Game (ADF&G;), Environmental Conservation (DEC),
and Division of Governmental Coordination (DGC) to site and construct your aquatic
farm project. This packet can be used to apply for the following: Aquatic
Farm Operation Permit and Special Area Permit (ADF&G;), Aquatic Farmsite Lease
(DNR), Coastal Zone Consistency Certification (COE) and General Permit 91-7.

Acquisition

Required when acquiring and/or transporting broodstock or seedstock
that is not from a State certified source (Sec. 16.40.120, Sec. 16.40.130, Sec.
16.40.150 and 5 AAC 41.070(b)).

Required for importing, exporting, and moving seed within the
State when the seed is from a CERTIFIED source (Sec. 16.40.130, Sec. 16.40.140,

Catcher/Seller
Permit Application Form — PDF file*

Required of commercial fishermen to sell their UNPROCESSED
catch off of their vessels to UNLICENSED buyers within the State
of Alaska. This permit is not valid for exporting unprocessed
catch out of the State of Alaska, nor does it allow for purchasing
fish from other fisherman. Other State and local requirements
may exist under certain conditions of this permit. Verify the
nature of your intentions with requirements outlined in the Processors
& Buyers (Intent to Operate - Seafood Processor and Exporter
License and Permit Application) before submitting this application.
Classroom Incubation Project Application Form (a type of Fish Resource Permit),
Classroom Incubation Project Report Forms, and School Vocational Incubation
Project Application Form — PDF file* (44K)
Required of classroom teachers who wish to take, transport, possess, and hold alive fish and their eggs
(except gold fish and decorative tropical fish) for a classroom or school incubation project.
The classroom project usually requires a recirculating aquarium for up to 500 eggs.
The vocational project usually requires a separate incubation facility for up to 50,000.
The review takes approximately 30 days.

This permit authorizes a person to transport and sell COMMERCIALLY taken salmon,
herring or Pacific cod as an agent of the commercial

Commercial Operator's Annual Report

Required by all seafood operations who filed an Intent to Operate
during the previous operating year (i.e. Buying, Processing, or Exporting).

Fish Resource Permit (FRP) Application Form (Sci-Ed Collecting Permit) — (January 2001)
Required of anyone who wants to collect or hold alive any live fish, shellfish, or aquatic plants or their gamets (except
gold fish and decorative tropical fish) for purposes of science, education, propagation, or exhibition.
A permit is valid for a calendar year; expiring December 31 each year. A new application and study/operational plan
is required to continue a study or project. The review takes approximately 30 days.
Available in the following formats: Word “Email” Form (Onscreen-fillable form — 76K) that can be emailed back to us, or a printable Word document (42K) or PDF file * (20K) that can be filled in by hand and then returned to us by standard mail.
Fish Resource Permit: Aquaria Attachment — (January 2001)
Required for the transport and live holding of specimens to be contained in aquaria.

Fish/Shellfish & Egg Transport Permit

Required of anyone who wants to transport, possess, export from the state, or
release into the waters of the state, any live fish. Applicants wishing to export
live fish from the state must attach a permit to import from officials of the
receiving state. In all cases the project must already have been discussed or
approved in another document or permit before an FTP application will be processed.
The review take approximately 45 days.
Fish Ticket Data Release Form —

Pursuant to Alaska Statute 16.05.815, individual fish ticket records are defined
confidential. Those requesting release of department records of their participation
in a given fishery must sign this form in the presence of a notary public or at
a local ADF&G; office. The notarized form may be turned in at any area office of
the Alaska Department of Fish and Game.
