     Guide Registration, Business Registration and Logbook Data Release

Instructions: Guide & Business Registration Forms (scroll down to download 2004 forms) Per 5 AAC 75.075(c) the Alaska Department of Fish and Game, Division of Sport Fish - RTS is responsible for overseeing the annual registration of Sport Fish Businesses and Guides.  Applications for the 2004 sport fish businesses and guides are available here for download in PDF format. Click on the link below for the desired form which will open in Adobe Acrobat Reader.

1. Print the applicable form(s), fill it out completely and legibly, and sign it.
   Incomplete forms will be sent back.
   
   
2. Send it to the address noted in the upper left portion of the form, or

3. Do NOT fax or e-mail this form. An original signature must be on file.
   
   
4. There is no fee.
   
   
5. Please allow 2 weeks for processing and receipt of a wallet-sized card.

IMPORTANT!: This form alone does not allow you to legally resume guiding or operating your guiding business, you must have the wallet sized card signed by a department representative before initiating any guiding services.

Instructions for Logbook Data Release Form

Pursuant to Alaska Statute 16.05.815, individual Saltwater Charter Vessel Logbook records are defined confidential. Business owner/agents requesting release of department records of their participation in a fishery using Saltwater Charter Vessel Logbooks must sign this form in the presence of a notary public. The Department provides an electronic summary of Saltwater Charter Vessel Logbook records. The Department cannot provide photocopies of Saltwater Charter Vessel Logbook records. The Department cannot provide vessel fishing history data. The notarized form may be submitted to any area office of the Alaska Department of Fish and Game, or sent directly to:
