Information & Requirements to Obtain a U.S. Passport
Service Charges and Fees

Applications for Passports are available only at the South County, North County, Mid-County, West County Court Houses and the Boca Annex.  Children 17 and younger MUST be present when applying for a new Passport or renewing Passports.

Hours of operation:

Our offices are open from 8:00 a.m. - 5:00 p.m. Monday - Friday excluding legal holidays. The Boca Annex is open Tuesday and Wednesday from 7:00 a.m. - 3:00 p.m.

All applicants must provide the following:

I. Proof of Citizenship

  1. Previous U.S. Passport -or-
  2. Certified copy of your Birth Certificate
  3. Certificate of Naturalization
  4. Minors 14 years and younger with a previous U.S. Passport must also submit long form, certified copy of Birth Certificate.  For children under 14 years of age, BOTH parents appear together and sign OR one parent appears, signs AND submits second parent's written statement consenting to passport issuance for child or children.
II. Two identical passport photographs (see back of application for specific requirements)

III. PROOF OF IDENTITY (provide one of the following)

  1. Previous U.S. Passport
  2. Valid Driver's License (a Temporary License or Driver's Permit do not meet this requirement)
  3. State Identification Card (photographic likeness should be recent)
  4. Certificate of Naturalization or of Citizenship FEES
IV. FEES

  1. Applicants 16 years of age or older

    1. Check in the amount of $55.00 payable to the Department of State
    2. A separate payment of $30.00 (cash or check) payable to the Clerk of the Circuit Court*

  2. Applicants 15 years of age or less

    1. Check in the amount of $40.00 payable to the Department of State
    2. A separate payment of $30.00 (cash or check) payable to the Clerk of the Circuit Court*

IF YOU CANNOT PROVE YOUR IDENTITY as outlined above, you must bring an Identifying Witness - a relative or someone who has a valid Driver's License, and has known you for at least two years. Your witness will be required to sign an Affidavit of Identifying Witness form.

* IF YOU HOLD A CURRENT PASSPORT (dated less than 15 years from the issue date), and you were 16 years of age or over when it was issued, you may withhold the $30.00 Clerk's fee by obtaining a renewal application and forwarding same directly to the Passport Agency.

FIRST TIME APPLICANTS

All first time applicants must complete and submit a DSP-11 application, show proof of citizenship and submit two identical passport photographs. Photos should be sufficiently recent to be a likeness of the Applicant (normally taken within the last 6 months), 2 x 2. inches in size, clear, front view, full face, they may be black and white or color. PROOF OF CITIZENSHIP CAN BE A PREVIOUS U.S. PASSPORT OR A CERTIFIED COPY OF THEIR BIRTH CERTIFICATE. Applicants 17 and younger MUST be present when applying for a new Passport or renewing Passports. When submitting proof of citizenship for a minor or someone 16 years of age or older that does not have a driver's license and is being identified by a parent, the large birth certificate with the parents names needs to be submitted. Proof of identity must be established. This can be a previous U.S. passport, certificate of Naturalization, driver's license (not temporary or learner's license), government identification card or pass.

  1. Applicants 16 years of age or older

    1. check or money order not cash in the amount of $55.00 payable to Department of State

  2. Applicants 15 years of age or less

    1. check or money order not cash in the amount of $40.00 payable to Department of State
    2. a separate payment of $30.00 (cash or check) payable to Clerk of Court

Passports are valid for ten years for Adults ( 16 years of age & older) and five years for children (15 years & younger)

EXPEDITED PASSPORTS:

Processing time: Total process could take 10-15 business days. Mailing and handling time in Pennsylvania excluded.

Fees: Applicants 16-years of age or older must pay $115.00 in a personal check, cashier's check, or money order to Department of State.

Applicants 15-years of age or younger pay $100.00.

A separate $30.00 processing fee is also required. Made payable in CASH or CHECK TO: CLERK OF THE CIRCUIT COURT.

You may also send the sealed application via mail or overnight delivery. If overnight delivery is used, you may want to consider having the passport returned to you by prepaid overnight delivery as well.

Change in policy regarding UPS, DHL, Federal Express, and Airborne: Over-night envelopes received for return delivery by the above companies must be prepaid or list a verified account number with the courier service.

PASSPORT AMENDMENT/VALIDATION APPLICATION
FORM DSP-19 (9-24)

You may request amendment/validation of your passport for the following reasons only:

* TO SHOW CHANGE OF NAME. Submit documentary evidence such as a certified court order, marriage certificate, or other satisfactory evidence to support a change of name.

* TO CORRECT THE DESCRIPTIVE DATA. Submit appropriate evidence to support correction of descriptive data.

* TO ADD VISA SUPPLEMENT PAGES.

* TO EXTEND THE VALIDITY OF A LIMITED PASSPORT. Submit appropriate evidence to support your request.

* IN CERTAIN CASES, TO SHOW ENDORSEMENT OR VALIDATION OF YOUR PASSPORT. Submit appropriate evidence.

How to apply for amendment/validation of your passport:

* Complete, sign, and date the passport amendment/validation application.

* Send the DSP-19 form with your passport and any required additional evidence to the nearest passport facility. You need NOT appear in person.

* There is NO FEE for this service. Your amended passport and any documentary evidence submitted will be returned to you by first-class mail. If you desire special postal service (registered, special delivery), include appropriate postage fees.

A passport agency may NOT mail a passport to a private address outside the United States.

Applicants for amendment/validation of official, diplomatic, or no-fee passports:

* Submit your U.S. Government or military authorization in addition to the items listed above. Consult your sponsoring Agency for instruction on proper routing procedures before forwarding this application. Your amended/validated passport will be released to your sponsoring Agency for forwarding to you.

RENEWAL APPLICATION BY MAIL

If you hold a current passport and it's not more than 5 years of expiration and not more than 15 years since being issued and you were at least 16 years of age or over when it was issue, you can renew your passport by mail. If the name you are currently using differs from the name on your most recent passport, you must submit a marriage certificate or court order showing the change of name. All renewal applicants must fill out a DSP-82 form. Your most recent passport must be mailed in with two new passport photos. Photos should be sufficiently recent to be a likeness of the applicant (normally taken within the last 6 months), 2 x 2 inches in size, clear, front view, full face, they may be black and white or color. ( If your passport is mutilated or damaged, you must apply on the DSP-11 application form and appear at one of the acceptance agency with a certified copy of your birth certificate). The application is then mailed to one of the following addresses listed on the back of the DSP-82 form.

  2. For expedited service, available only in the United States. Requests will be processed in 3 workdays from receipt at the Passport Agency. This service is available for early departure only, generally with proof of travel tickets or other proof of necessity to travel. The additional fee is $60.00.
