Club Car Announces Recall to Repair Golf Cars and Other Vehicles - January 13, 2005 - Club Car is recalling 19,663 2004 and 2005 model year gasoline and electric powered Golf Cars and other vehicles. Microscopic cracks in the brake equalizer rods on certain vehicles may cause the rods to fatigue and fail prematurely in continuous use. Such failure could result in a loss of braking ability. The recalled vehicles are used for short distance transportation around golf courses, hotels, resorts, airports, and residential communities. Read more...

American Suzuki Motor Corporation Announces Recall of 2005 DR-Z110K5 Off-Road Motorcycles - December 10, 2004 - Suzuki is recalling about 1,500 2005 model year DR-Z110K5 Off-Road Motorcycles. The rear shock absorber may have been assembled with an incorrect o-ring, allowing fluid to leak from the shock absorber. Fluid loss from the shock absorber can compromise the shock's damping characteristics, which can render the motorcycle unstable creating a potential for injury or death. This motorcycle is designed for a rider weighing less than 121 pounds. Suzuki dealers sold these ATVs nationwide between July 2004 and early November 2004. They are yellow in color with red stripe on the side of the fuel tank with a large "S", the Suzuki logo. Read more...

Kawasaki Motors Is Recalling 2005 Off- Highway Motorcycles - December 10, 2004 - Kawasaki is recalling about 1,500 2005 model KLX110-A4 off-highway motorcycles. On some units, the rear shock absorber may have been assembled with an incorrect o-ring, allowing fluid to leak from the shock. Fluid loss from the shock can compromise the shock's damping characteristics, which can render the motorcycle unstable creating a potential for an injury or death. The recalled Kawasaki motorcycles are "minicycles," small-sized motorcycles intended for use by minors and adults. Polaris Industries Announces Recall of 900 Fusion, 900 RMK Snowmobiles - December 07, 2004 - Polaris is recalling about 2,580 "900 Fusion" and "900 RMK 151" snowmobiles. The fuel connections between the fuel line and the fuel pump and fuel filter can leak. Failure of the fuel connection can result in leaking fuel and a possible fire, which can result in severe injury or death. In addition, the drive shaft may have been incorrectly heat treated resulting in brittle drive shafts that could fail at lower loads than the shafts were originally designed to meet. Failure of the drive shaft can result in a loss of braking of the snowmobile and severe injury or death. Model year 2005 Polaris "900 Fusion" and "900 RMK 151" snowmobiles with model numbers S05MP8DS(A)(B)(C) and S05PL8DSA are part of this recall. The model number is located on the vehicle identification tag located on the right side near the foot rest. The "900 Fusion" and "900 RMK 151" snowmobiles have either red/black or blue/silver/black seats with a red, black, or blue hood. American Suzuki Motor Corporation Announces Recall of 2004 & 2005 Eiger™ and Vinson® ATVs - December 07, 2004 - American Suzuki is recalling about 27,000 2004-2005 Eiger and Vinson ATVs. The recalled ATVs were assembled with an incorrectly sized mounting bolt under the fuel tank. This could result in fuel leakage presenting a fire safety hazard and risk of injury or death. Jumpking ® Announces Recall of Trampolines, Enclosures - January 25, 2005 - Jumpking Inc.® is recalling about 1 million trampolines and about 296,000 "FunRing" enclosures sold separately and with Jumpking trampolines. Welds on the frame of these trampolines can break during use, resulting in falls and possible injuries. Additionally, the mounting brackets of the FunRing enclosures have sharp edges, which can cause lacerations. The brand name "Jumpking, Inc." is written on a warning label wrapped around a leg of the trampoline. Read more...

Pottery Barn Kids Announces Recall of Chamois Blankets - January 24, 2005 - Pottery Barn Kids is recalling about 92,000 Chamois Blankets. The decorative stitching on the blanket’s edge can come loose, allowing a child to become entangled in the yarn. This poses a strangulation hazard to young children. This recall involves children’s chamois blankets made of 100% polyester. The blankets were sold in yellow, pink, blue and green, and the reverse side of all the blankets are white. Decorative yarn in a coordinating color is stitched around the blanket’s edge. The blanket measures 30-inches wide by 40-inches long and are labeled for stroller use. A label on the blanket reads "pottery barn kids" and "Made in Korea." Read more...

Kids Station Announces Recall of Children’s Musical Drum Set Sold Exclusively at Toys 'R' Us - January 19, 2005 - Kids Station is recalling 10,500 Fun Years Music Big Drum Musical Sets. Small parts can break off during use, posing a choking hazard to young children. The recalled music set includes a yellow and green drum with a clear plastic lid and a red one-piece carrying strap that doubles as a storage and carrying case for additional instruments. The instruments stored in the drum include: purple drumsticks, yellow and red dumb-bell-shaped maracas, purple wrist bells, an orange recorder, a yellow harmonica, a red-rimmed tambourine and a yellow whistle. Riviera Trading Announces Recall of Children's Costume Bracelets - January 11, 2005 - Riviera Trading is recalling about 7,100 Metallic costume bracelets. The bracelets contain high levels of lead and pose a lead poisoning hazard to children. The recalled bracelets have heart, oval, and rectangular shaped charms that have the phrases; "I like sports," "I like movies," "I like shopping" and "I like music" printed on them. Orbelle Trade Announces Recall of Cribs - January 06, 2005 - Orbelle Trade is recalling about 7,700 cribs. The Amber model crib included in this recall does not comply with crib safety standards due to a gap between the side rail and the crib mattress support, posing an entrapment hazard to infants. All other model cribs included in this recall do not have proper assembly instructions and diagrams required for cribs. They are also missing cautionary and warning labels as required by federal law. If the cribs are not assembled properly they could pose an entrapment hazard to infants. The cribs included in this recall are the Daniella, Leeat, Noa, Naomi, Gabriella, Amber and Series 300 model cribs. The cribs are wood and come in a variety of painted and stained finishes including natural, cherry, blonde, or white wood. Step 2 Co. Announces Recall to Repair Toddler Swings - January 04, 2005 - Step 2 is recalling about 9,300 Toddler Swings. The straps on these swings could break, causing a child riding in the swing to fall to the ground and suffer injuries. The recall includes toddler swings with model numbers 7336KR and 733604. The model numbers are located on a label on the back of the swings. The swings are made from green plastic and the red Step 2 logo is located on the front of the swing. AA Importer Announces Recall of Push and Electronic Toys - December 28, 2004 - AA Importer is recalling about 5,200 Push Along Frog, Animal Organ, and Rock & Roll Kids Guitar Toys. The toys can easily break apart, exposing small parts. This poses a choking hazard to young children. The Push Along Frog toy has a 9.5- inch green plastic frog attached to a 15-inch, red plastic handle. A sticker on the front of the toy reads "Push Along Forg" (sic). The electronic Animal Organ comes in four different styles, all about 11- inches long. The style number, written on the back of the toy begins with "666." The electronic guitars are about 14-inches long and have various colored buttons. "ROCK & ROLL KIDS" is written on stickers on the front. "MADE IN CHINA" is written on all three toys. Read more...

Raymond Geddes Announces Recall of Children's Necklaces - December 17, 2004 - Raymond Geddes is recalling about 155,000 Metallic necklaces. The necklaces contain high levels of lead. CPSC regulations ban children's products from containing high levels of lead due to the risk of lead poisoning resulting from contact with these products. The necklaces also contain a sharp point, posing a laceration hazard to young children. The recalled necklaces have medallions that came in four different designs depicting frogs, dolphins with a small bead on the tail, a sunshine smiley face and an alien face on a starburst. Norton McNaughton Announces Recall of Girls' Hooded Sweaters with Drawstrings - December 09, 2004 - Norton McNaughton is recalling 157,300 Energie Brand Hooded Sweaters. The hooded sweaters have drawstrings, posing a strangulation hazard to children. The recalled hooded drawstring sweaters were sold in a variety of colors and youth sizes S, M, L and XL. They were sold in either the pull-over hooded style or with a zipper. The RN Number 33364 and the word "energie," together with a small red flower, are printed on the collar tags. Read more...

Country Home Products Announces Recall to Repair DR® Leaf and Lawn Vacuum - February 01, 2005 - Country Home Products is recalling 1,071 DR® Leaf and Lawn Vacuums. These leaf and lawn vacuums have a plastic elbow and blower exit adaptor that is connected to the top of the shredding chamber. The plastic can crack allowing stones, large sticks, and other material to puncture the plastic. This creates the possibility that pieces of plastic or other debris can fly upward toward bystanders or the operator. When this occurs, the potential for personal injury or property damage exists. Rossignol and Dynastar Announce Recall of Snowboard Bindings - February 01, 2005 - Rossignol and Dynastar are recalling about 11,000 pairs of snowboard bindings. The baseplate of the binding can break, resulting in loss of control and possible fall and injury to the snowboarder. This recall includes all 2003/2004 model year Rossignol-brand HC 500, Zena, Unit Pack, HC Rental, and Unit Rental snowboard bindings. Polk Audio Announces Recall of LSiW Powered Subwoofers - January 26, 2005 - Polk Audio is recalling about 670 LSiW Powered Subwoofers. Electrical components in the subwoofer can overheat and fail, resulting in electrical arcing and smoke. This recall includes Polk Audio model LSiW subwoofers with part numbers AM8080-A (Ebony enclosure) and AM8083-A (Cherry enclosure). Affected subwoofers have a serial number from AM808000100 to AM808000597 for the Ebony enclosure and AM808300100 to AM808300599 for the Cherry enclosure. Lakewood Announces Recall of Oil- Filled Electric Heaters - January 26, 2005 - Lakewood is recalling 70,000 Model 5101oil-filled electric radiator heaters. Welds can rupture, expelling hot oil that can burn nearby consumers. The units are grayish-white in color and have the name "Lakewood" printed below the handle indentation on the control panel. Research Products Corp. Announces Recall of Electronic Air Cleaners - January 24, 2005 - Research Products Corporation is recalling about 67,000 Aprilaire Electronic Air Cleaners. The air cleaner's plastic inner housing and filter are not flame-resistant. If electrical arcing occurs in the cleaner, a fire can develop. This poses a risk of burn injuries and property damage. The recall involves all Aprilaire Model 5000 Electronic Air Cleaners. The air cleaners are almond colored, and located near heating and cooling equipment. Carlton Cards Announces Recall of Tealight Candle Holder - January 21, 2005 - Carlton Cards is recalling about 5,000 Roasting Marshmallow Tealight Candle Holders. During normal use, the decorative marshmallows or decorative campfire flames may catch fire. The candle holder is a Christmas decoration designed to hold a tealight candle. Selle Royal Announces Recall of Bicycle Seats - January 14, 2005 - Selle Royal is recalling about 257 Bicycle Seats. The plastic scuff guard underneath the front of the seat, which secures the seat rails, may crack or break causing the rider to lose control and/or fall, resulting in a serious injury. The recall involves Selle Royal bicycle seat part number 7403SXS 03 9072 with a manufacture date of 10/03. The bicycle seat is an original equipment seat installed on 2004 model year Recreational Equipment Inc. (REI) brand Novara Express bicycles. Wal-Mart Stores Announces Recall of Slow Cookers - January 13, 2005 - Hamilton Beach/Proctor Silex is recalling 600,000 GE Oval and Double Dish Slow Cookers. The handles on the base of the slow cookers can break, posing a risk of burns from hot food spilling onto consumers. The recalled slow cookers include the 4.5-Quart Oval and 6-Quart Double Dish models sold under the General Electric (GE) brand name. The model name is printed on the front of the units. The 4.5-quart unit has model number 106661 and the 6-quart unit has model number 106851. Both models have series codes A through D, which are printed on the bottom along with the model number. Sauder Woodworking Announces Recall of Mini Spot-light Bulbs in Furniture Hutches - January 13, 2005 - Comtrad is recalling about 22,600 40 Watt Incandescent Mini Spot-light Bulbs. Some of the bulbs have imploded or broken apart within the first several minutes after they are first illuminated. Broken glass particles pose a laceration hazard to consumers. The recalled 40 Watt "Type R" incandescent mini spot-light bulbs were included in two- and three-lamp accent lights in certain furniture hutch items manufactured by Sauder Woodworking. Climb High Announces Recall of Avalanche Transceivers - January 13, 2005 - Ascom Systec is recalling about 137 Mammut Barryvox Avalanche Transceivers. This is a device worn by backcountry skiers, and used to help locate a buried avalanche victim. The transceivers’ plastic casing could crack, causing a malfunction if water were to enter the unit. If the transceiver fails to function properly in the aftermath of an avalanche, it could result in the buried victim not being found in time to avoid serious injury or death. The recalled Barryvoxes are 2004/2005 model Opto 3000 Avalanche Transceivers. The words, "Mammut" and "Barryvox" are printed on the front of the red and black transceivers. The serial numbers on the recalled units, which can be found in the battery compartment under the UPC code, include: M0080000 through M0088419, M0089616 through M0089800 and M0090000 through M0090419. SHURflo Announces Recall to Repair Backpack Sprayers - January 11, 2005 - SHURflo is recalling 6,100 ProPack Rechargeable Electric Backpack Sprayers. The power plug can corrode after extensive exposure to water and corrosive chemicals, posing a fire hazard to consumers. The recalled SHURflo ProPack Model SRS-540 rechargeable electric backpack sprayers have manufacture dates before September 2004. The model number and manufacture date are printed on a label affixed to the motor bracket, which can found by removing the battery pack. Norco Products Announces Recall of Bicycle Attachment - January 07, 2005 - Norco Products is recalling about 80,000 Adams Trail-a-Bike and Adams Slipstream bicycle attachments. The hardware used in the assembly of the universal hitch may come loose causing the unit to separate from the lead bicycle, which could lead to a serious injury or death to the rider of the bicycle attachment. The recall involves the Adams Trail-A-Bike or Adams Slipstream sold under the following models: Starter, Folder, Shifter, Shocker, Ultimate Tandem, Deluxe Folder, Me 2 and Original. Crosman Corporation Announces Recall of Air Pistols - December 22, 2004 - Umarex (manufacturer) and Crosman Corporation (distributor) are recalling about 24,000 Walther CP Sport, CP99, and Nighthawk CO2 Air Pistols. The air pistols’ safety might move from the engaged or "safe" position to the disengaged or "fire" position when the trigger is pulled once or more and then released. Robertshaw Controls Company Announces Recall of Certain Models of 7000 Series Gas Control Valves - December 22, 2004 - Robertshaw is recalling about 425,000 7000 Series Gas Control Valves. If the pilot light goes out, the gas valve could stick in an open position, permitting gas to continue to flow. This can result in a gas explosion and fire, which could result in severe personal injury or property damage. The 7000 Series Gas Valves that are being recalled are installed in gas appliances including residential space heaters, wall heaters, boilers, fireplaces, pool heaters, infrared heaters and furnaces, and commercial heating applications such as commercial cooking appliances, fryers, commercial water heaters, and poultry brooders. The recalled valves were manufactured between February 2003 and September 2004 and have production dates beginning with code 0306 and ending with 0436. However, not all 7000 Series valves manufactured during the indicated timeframe are being recalled. Of particular note are model numbers containing the letters "MV." Recalled gas valves include "MV" models that also include the letters "LP" and model numbers 7000AMV; 7000BMV; 7000BMV-S7CL; 7000MVLC; and 7000MVRCLC. All "MV" valves with the indicated date codes that have been converted to Liquid Propane (LP) gas use are also included in the recall. NOT included in the recall are model numbers 7010 and above; model numbers containing the letters "D" or "BV;" and model numbers containing the letters "MV" except for the ones listed above. GE Security Announces Retrofit/Recall of Smoke Detector Systems - December 22, 2004 - GE Security is recalling approximately 246,800 smoke detector systems. The smoke alarms can fail to activate during a fire or emergency, if installed in combination with certain control panels. The recalled smoke detection systems involve hard-wired S10A ESL smoke detectors that are incompatible with certain ITI control panels as well as certain S09A/S10A listed ESL smoke detectors that may be incompatible with control panels from other manufacturers. Swing-N-Slide® Corp. Announces Recall of Extra-Duty and Heavy-Duty Swing Seats - December 16, 2004 - Swing-N-Slide Corporation is recalling 72,000 Extra-Duty and Heavy-Duty Swing Seats. A manufacturing defect can cause the metal grommet securing the seat to break. If this happens, the user can fall to the ground. Sold in either green (extra-duty) or yellow (heavy-duty), the seats are suspended from a crossbeam by two plastic-coated metal chains. First Team Announces Recall of Basketball Hoops - December 14, 2004 - First Team is recalling 7.600 basketball hoops. The bolt attaching the arm to the vertical post can break, causing the backboard to fall and injure someone standing below. This recall involves First Team Attack, Force and Titan basketball hoops. The hoops have a spring-loaded E-Z crank, adjustable backboard that goes from 6-feet, 6-inches to 10-feet tall. SCUBAPRO Announces Recall of Converter Kit - December 09, 2004 - SCUBAPRO is recalling 3,300 Converter for Air 2 Alternate Inflation Regulators. Technical Consumer Products Announces Recall of Fluorescent Light Bulbs - December 08, 2004 - Technical Consumer Products is recalling 158,000 3-way compact fluorescent light bulbs. The bulbs can overheat and spark, posing a burn hazard to consumers. The recalled 32-watt, 3-way (40-75-150 watt output) compact fluorescent bulbs were sold under the brand names Commercial Electric (Home Depot) and DuraBright (Orchard Supply Hardware). The bulbs are about seven inches high and have a white, spiral fluorescent tube attached to a white plastic base. Robert Bosch Tool Corp. Announces Recall of Skil® Table Saws - December 06, 2004 - Bosch Tool Corporation is recalling 120,000 Skil® Table Saws. The blade drive mechanism may loosen or the motor can separate from the tool. Loosening of the blade drive mechanism can result in kickback of the item being sawed, resulting in possible laceration. Motor unit separation can cause the coasting saw blade to damage the saw wiring resulting in possible electric shock, or the separated motor could strike the user and cause injury. Only Skil® table saws with model number 3400 printed on the front side of the table base with the date codes listed below are included in the recall. Date codes are printed on the upper right corner of the table base and include 2002 date codes 28501-28831, 2003 codes 38101- 39231 and 2004 codes 48101-48811. Lowe’s Announces Recall of GardenPlus Industrial Carts - December 06, 2004 - LG Sourcing and Lowe's have announced the recall of 196,788 GardenPlus Industrial Garden Carts. Over- inflation of the tires can break the metal tire rim, causing users and bystanders to be struck with pieces of metal. The recalled GardenPlus Carts, item number 128483, were made between November 2002 and April 2004. Electrolux Home Products Announces Recall of Husqvarna Lawn Tractors - December 06, 2004 - Electrolux Home Products is recalling 5,280 Husqvarna Lawn Tractors. These lawn tractors can develop abrasions on the fuel tank because of the fuel line clamp’s location. This can result in a fuel tank leak, which could pose a fire hazard to consumers. These Husqvarna 18.5 horsepower, hydrostatic transmission, 42-inch cutting deck, lawn tractors are gasoline-powered and are designed for residential use. Alliant Pharmaceuticals Issues Voluntary Nationwide Recall of One Lot of Methylin® CT, 5mg - January 14, 2005 - Alliant Pharmaceuticals, Inc. is voluntarily recalling one lot of Methylphenidate HCl Chewable Tablets, 5 mg strength, lot number #AMT50402A. After testing and evaluation, Alliant found that this lot of 5 mg strength might contain up to three times the active ingredient, and elected to recall the medication as it could pose serious health risk for some patients. Methylphenidate HCl Chewable Tablets (Methylin® CT) are a therapy for Attention Deficit Hyperactivity Disorder and Narcolepsy. Alliant immediately notified the FDA of its findings, and is notifying doctors and pharmacists of the recall by letter. They will be asking pharmacists to notify patients who purchased the recalled product from their pharmacy. Distributors and pharmacies should check the lot numbers on the product label and promptly quarantine any product with the lot number #AMT50402A and expiration date of April 2006. Read more...

Nationwide Warning/Recall on Hospitak Airway Adapters Expands to Other Lots and to Viasys, Dräger, and Unomedical Brand Airway Adapters - December 02, 2004 - Unomedical Inc., of McAllen , Texas , announced that it was expanding its warning and beginning a recall of airway adapters it distributed in the U.S. and Europe . Several adapters have been found to be blocked or occluded - potentially preventing exhalation or inhalation. This could result in serious or life-threatening injury to patients. Subsequent investigation by the company indicates that other lots of the product, including airway adaptors used in other medical devices, may have been affected. Some of the affected products have been labeled and sold under the Viasys, Unomedical, and Dräger brand name. The total list of affected products and lot numbers are as follows.
