Driver License Office Locations

The following office locations offer Driver Records, Crash Reports, and Driver License Reinstatements:

Reinstatement Fees

Suspended/cancelled licenses $100
Revoked licenses $175

Alcohol/drug-related offenses:

Suspended $275
Revoked $275
Additional drug-related fee $25

Other Fees:

Fail to surrender license within 30 days $50
Fail to pay child support $50

Driver License Reinstatements must be performed at one of the Driver License Offices listed above.

Driver Record Fees

Copy of Driver Record (MVR) $5.75

Driving Records may be purchased through the mail using the Driver Record Request Form, or in person at any of the Driver License Offices listed above.

Crash Report Fees

Copy of Crash Report $15

Driving Records may be purchased through the mail using the Crash Report Request Form (PDF format, 18 KB), or in person at any of the Driver License Offices listed above.

NOTE: NO PERSONAL CHECKS ARE ACCEPTED.
