LEVEL TERM ASSURANCE PLAN

ITS AIMS

To provide a tax free cash sum for your dependants on your death within a specified period of years.

YOUR COMMITMENT

You must answer all the questions in the application correctly to the best of your knowledge and belief. If you do not do this, your plan will not pay out.

You agree to pay a regular monthly premium until the end of the policy term or your earlier death.

RISK FACTORS

If you stop paying premiums the life cover will stop 15 days after the due date of the last premium you paid.

The plan has no cash-in value at any time.

If you live to the end of the policy term, the policy ceases without any value

We may not pay out or your cover may be reduced in the circumstances described under “What exclusions or limitations apply to the life cover?” on page 2.

HOW WILL THE PLAN WORK FOR ME?

The premium depends on your personal circumstances – your age, sex and whether you smoke – the amount of cover you choose, and how long you decide you want the cover to last.

Your personal quotation shows the monthly premium for the amount and period of life cover you have chosen.

WHAT IS A TERM ASSURANCE PLAN?

A Term Assurance Plan is designed to provide you with life cover for a specified number of years.

The policy does not build up any value, there is no cash return if you stop paying premiums or if you survive to the end of the term.

You must be aged between 18 and 49 at the start of the plan and resident in the United Kingdom.

IS THE LIFE COVER GUARANTEED?

Yes, subject to the exclusions and limitations stated below. Provided you maintain the premiums, and assuming there are no gaps in the information given to the company your dependants are guaranteed to receive the life cover provided by the plan when you die.

WHAT EXCLUSIONS OR LIMITATIONS APPLY TO THE LIFE COVER?

If it is found you were over age 49 when you started your policy we will only be liable for the return of the premiums paid (see condition 7 of the Policy).

If any levy is imposed on us under the Financial Services Compensation Scheme or if any other legislation or Inland Revenue ruling affects our ability to pay the life cover, we reserve the right to reduce it (see condition 8 of the Policy).

HOW DOES MY ESTATE CLAIM ON MY DEATH?

A claim must be made to us at The Great Hall, Tunbridge Wells, Kent TN1 1RG.

To enable a claim to be paid we shall require proof of:

your death

your date of birth

entitlement of the person claiming payment

We shall also require the policy document and a completed claim payment form.

We may require medical or other reports from your doctor or other medical attendant.

FURTHER INFORMATION

CANCELLATION RIGHTS You can cancel your plan within 30 days of the commencement date of your policy. If you cancel within this period you will receive a refund of any premiums you have paid.

If you wish to cancel your plan please write to Reliance Mutual Insurance Society Limited, The Great Hall, Mount Pleasant Road, Tunbridge Wells, Kent TN1 1RG or email us at contact@reliancemutual.co.uk.

If you do not cancel within this period you can cancel the plan at any time by ceasing to pay further premiums, but you will not be entitled to a refund of any premiums you have paid and your plan will cease without value.

PREMIUM Your first premium is paid by credit card and subsequent premiums must be paid by Direct Debit. Missed premiums will mean that your life cover ceases.

CHARGES All charges and expenses are taken into account in the premium for the plan.

SURRENDER/PAID-UP POLICY The policy does not acquire a surrender or paid-up value.

TAX There are taxes on the assets backing the policy benefits, but these are taken into account in the premium for the plan. The death benefit may be subject to inheritance tax. Further details are available on request.

LAW In legal disputes Reliance Mutual has chosen to use the Law of England.

LANGUAGE All communications will be in the English language.

QUERIES AND COMPLAINTS For further information or if you wish to complain about any aspect of the service you have received, please contact Reliance Mutual Insurance Society Limited at the address given in this document. If your complaint is not dealt with to your satisfaction you can complain to the Financial Ombudsman Service, South Quay Plaza, 183 Marsh Wall, London E14 9SR. Telephone: 0845.080.1800. There are some instances where the Financial Ombudsman Service cannot consider your complaint. Making a complaint will not prejudice your right to take legal proceedings.

COMPENSATION Should Reliance Mutual Insurance Society Limited become unable at any time to meet claims against it, your interests will be protected by the Financial Services Compensation Scheme. You will be covered in full for the first £2,000 of a claim under your plan and 90% of the balance.
