Active collection of demographic information is requested solely to assist the Amputee Coalition of America in carrying out its missions of outreach, empowerment, and education, and to support the aims of the Limb Loss Research and Statistics Program.

Parents may review their child’s personal information, have it deleted, or refuse to permit any further collection by contacting the ACA at the address below.
