Note: If you have recently registered your new vehicle, please call the DMV at 860-263-5154 with your current marker plate number to avoid delays in processing.
Every attempt will be made to accommodate your request. However, the Department of Motor Vehicles reserves the right to deny issuance of certain requests.

Two copies of this completed form must be submitted to the DMV. Without two copies, the DMV will be unable to process your request. Rejected requests will require you to resubmit an inquiry for a specific plate.
