"Health Care Is An Essential Safeguard Of Human Life And Dignity And There Is An Obligation For Society To Ensure That Every Person Be Able To Realize This Right."Physicians for a National Health Program is a not-for-profit organization of physicians, medical students, and other health care professionals that support a national health insurance (NHI) program. Specifically, we believe that a single-payer system (where the government finances health care, but keeps the delivery of health care to mostly private control) is the only solution to solving the United States' many health care problems: 43 million citizens with no health insurance, many more with only limited coverage, skyrocketing health insurance premiums, malpractice costs, long-term care issues, and relatively poor health indicators, when compared to similar industrialized nations. Directory: PNHP has already played a critical role in building public awareness of the single-payer alternative. We published the first major single payer proposal in the New England Journal of Medicine in 1989. Our plans for quality improvement, financing, and for reforming long-term care, published in the Journal of the American Medical Association, sparked discussion and media coverage. PNHP members have addressed hundreds of grand rounds and conferences, testified before dozens of federal and state legislative committees, spearheaded single-payer efforts in several states, and worked extensively with the media and local community groups to educate the public. Volume 8 is the last volume for the foreseeable future. There are at present no plans to continue the series in either the existing or a newer format. Effective Health Care bulletins are based on a systematic review and synthesis of research on the clinical effect, cost-effectiveness and acceptability of health service interventions. This is carried out by a research team using established methodological guidelines, with advice from expert consultants for each topic. Recent study finds that the quality of care received by Medicare beneficiaries varies widely across areas and that states with higher Medicare spending have lower quality care. This negative relationship may be driven by the use of intensive, expensive care that crowds out the use of more effective care. Professor Wen Hai suggests that in the reform of marketization, it is necessary to enhance government responsibility for public health and medical care, increase government investment in public health and rural medical care. The urbanization process in eastern regions is faster than that in middle and western regions, and a large portion of rural population has transferred into cities. Initiatives include systems for computerized physician order entry, E-prescriptions, wireless bedside bar-coded medication, and electronic medical records. They're able to flag potential problems such as drug allergies and dangerous interactions, enable critical-care personnel to treat patients remotely, and provide time- and cost-saving solutions.
Technology also is helping companies deal with a nationwide shortage of intensivists or critical-care doctors. Saint Luke's Health System in February deployed an electronic intensive-care unit that lets it provide round-the-clock monitoring of as many as 100 ICU patients by one intensivist physician and two critical-care nurses at a remote center, says John Wade, Saint Luke's VP and CIO. The E-ICU doctor and nurses observe patients via cameras. Monitors are programmed to send alerts if a patient's condition changes or other problems develop. Remote doctors can adjust some of the equipment from afar, such as the speed of intravenous drips, Wade says. Remote nurses and doctors also instruct on-site personnel on patient care. Without the E-ICU, Saint Luke's would have had to close many of its ICU beds at its rural facilities in Kansas and Missouri, Wade says. "There's a shortage of about 35,000 to 40,000 intensivists nationally, yet government guidelines require an intensivist at every ICU bedside," he says. "Technology is helping to bridge that gap by bringing board-certified intensivists to the bedside electronically." Baylor Health Care System, which operates hospitals in the Dallas/Fort Worth area, has introduced MediKiosks, wireless "E-clipboards" used by patients registering at the company's breast imaging center, says Bob Pickton, Baylor's senior VP and CIO. MediKiosks simplify a process that had required 13 paper forms and save 18 hours a day in staff time. They will be rolled out at 15 other outpatient facilities over the next three years, he says. Health CareIntroduction One Supreme Court decision may have done more to change health care in Canada than three major reports and a first ministers conference that ended with a $41-billion infusion into the system. "Access to a waiting list is not access to health care," two of the justices wrote in their decision. Still, the ruling could eventually lead to some of the biggest changes since former Saskatchewan premier Tommy Douglas was credited with fathering medicare. Most Canadians take government-funded health care for granted today, but when it was first introduced in Saskatchewan in 1962, most of the province's doctors responded by going on strike to protest against "creeping socialism." The strike lasted three weeks - public support for the doctors had collapsed, persuading the doctors to accept a deal with the government. Within five years, government-funded health care spread across the country. While most Canadians - 80 per cent according to Statistics Canada - are satisfied with their access to the health care system, many experience long waits to see a specialist, get diagnostic tests and undergo elective surgery. Others find themselves facing huge bills for prescription drugs they need to survive. With Romanow's landmark report under their belts, the nation's first ministers gathered in Ottawa in February 2003 for a meeting that was described as the most important session on health care since Canada adopted medicare. The prime minister, premiers and territorial leaders got together to try to turn some of Romanow's recommendations into action. To a large degree, the 2004 federal election turned into a debate about the future of health care in Canada. The Liberals accused the newly united Conservatives of plotting to turn medicare into a two-tiered system where those who could afford to pay more would be able to buy speedier access to the system. Less than two months after the election, Prime Minister Martin convened a first ministers conference on health care. It was to last three days. Health Care Financing Review Conference Proceedings Disease ManagmentIn December 2004, the Bertelsmann Stiftung, The Commonwealth Fund, and AcademyHealth jointly sponsored a conference in Berlin, Germany on "Case Studies in Chronic Care Management." This conference assembled representatives from the U.S. and German Governments, several organizations that had developed chronic care management initiatives in the two countries, and other health care and policy experts to discuss clinical, organizational, and financing issues. The case studies discussed at the conference are presented in this issue of the Review; this article describes some relevant considerations that are common to the U.S. and German health systems.
Disease management has been defined as a system of coordinated health care interventions and communications for populations with conditions in which patient self-care efforts are significant (Disease Management Association of America, 2005). The purpose of this article is to provide an overview of the diabetes disease management program offered by American Healthways (AMHC) and highlight recently reported results of this program The first disease management program contract for breast cancer in Germany was signed in 2002 between the Association of Regional of Physicians in North-Rhine and the statutory health insurance companies in Rhineland. At the heart of this unique breast cancer disease management program is a patient-centered network of health care professionals. The program's main objectives are: (1) to improve the quality of treatment and post-operative care for breast cancer patients, (2) to provide timely information and consultation empowering the patient to participate in decisionmaking, (3) to improve the interface between inpatient and outpatient care, and (4) to increase the number of breast-conserving surgeries. Hospital admissions among patients with congestive heart failure (CHF) are a major contributor to health care costs. A comprehensive disease management program for CHF was developed for private and statutory health insurance companies in order to improve health outcomes and reduce rehospitalization rates and costs. The program comprises care calls, written training material, telemetric monitoring, and health reports. Currently, 909 members from six insurance companies are enrolled. Routine evaluation, based on medical data warehouse software, demonstrates benefits in terms of improved health outcomes and processes of care. RationalMed Patient Safety Solutions, which analyzes clinical data from various
sources to identify potential drug-related problems. In the last year, the system
identified nearly a million patients at risk for medication issues, says Mark
Halloran, Medco's senior VP and CIO. Medco developed proprietary clinical rules
for the system that are applied to its NCR Teradata 30-terabyte data warehouse,
which, Halloran says, is the 23rd largest data warehouse in the world.
Germany
and the U.S. share a keen interest in exploring the potential of care management
programs for the chronically ill. Despite obvious health system differences, in
both countries there has been a proliferation of disease management models, initiated
by a variety of actors, paid for in different ways, targeting different types
of population groups, and encompassing a broad menu of interventions and services.
Comparison of three case studies from the U.S. and four from Germany reveals greater
differences among models within countries than between them.
