Two- The type o treatment is less critical than the individual’s commitment to change. People can select how they want to pursue change in line with their own values and preferences. They don’t need to be told how to change.

Three- Brief treatments can change longstanding habits. It is not the intensity of the treatment that allows people to change but rather its ability to inspire continued efforts in that direction.

Four- Life skills can be the key to licking addiction. All addictions may not be equal; the community reinforcement approach, with its emphasis on developing life skills, might be needed for those more severely debilitated by drugs and alcohol.

Five- Repeated efforts are critical in change. People do not often get better instantly—it usually takes multiple efforts. Providing follow-up allows people to maintain focus on their change goals. Eventually, they stand a good chance of achieving them.

Six- Improvement, without abstinence, counts. People do not usually succeed all at once. But they can show significant improvements; and all improvement should be accepted and rewarded. It is counterproductive to kick people out of therapy for failing to abstain. The therapeutic approach of recognizing improvement in the absence of abstinence is called harm reduction.

Alcoholism is a chronic, progressive, and often fatal disease. It is a primary disorder and not a symptom of other diseases or emotional problems. The chemistry of alcohol allows it to affect nearly every type of cell in the body, including those in the central nervous system. After prolonged exposure to alcohol, the brain becomes dependent on it. The severity of this disease is influenced by factors such as genetics, psychology, culture, and response to physical pain.

The following categories of alcohol use and abuse use a definition of one drink as 12 ounces of beer, 5 ounces of wine, or 1.5 ounces (a jigger) of 90-proof liquor.

Moderate Drinking. Moderate drinking, particularly red wine, appears to offer health benefits. Moderate drinking is defined as equal to or less than two drinks a day for men and equal to or less than one drink a day for women.

Physicians in the emergency department (ED) regularly encounter patients seeking treatment for alcohol or substance abuse problems. The initial evaluation may seem routine, yet these patients have multiple physical and emotional issues that should be addressed. The emergency physician (EP) should strive to identify patients who might benefit from appropriate referrals for drug and alcohol problems. The ED may be the initial or only point of contact with the health care system for these patients.

Substances of abuse include alcohol, cocaine, opiates, amphetamines, and hallucinogens. This chapter provides a brief review of the physiologic effects of these substances as well as the signs and physiologic effects of withdrawal with which the caregiver should be familiar. The Tough Decisions Behind Hormone Therapy

Women have relied on hormone therapy for decades to relieve symptoms of menopause. But with last year's dramatic the national institutes of health studyannouncement about the risks of hormone therapy, women suddenly faced an agonizing choice: Was relief from hot flashes really worth the increased risk of heart disease or breast cancer?

The National Institutes of Health study, known as the Women's Health Initiative, found that the most common form of hormone therapy, estrogen-progestin pills, significantly increased the risk of stroke, heart disease, and breast cancer.

NIH researchers concluded that these risks far outweighed the pills' benefits of preventing hip fractures and colorectal cancer, especially since other non-hormone treatments are available to safely fight bone loss and colon cancer.

Specifically, researchers found that women taking the estrogen-progestin combination are at much higher risk of cancer and heart disease than previously thought. However, on the positive side, the same group of 10,000 women will have six fewer cases of colon cancer and five fewer hip fractures.

"The cardiovascular and cancer risks of estrogen plus progestin outweigh any benefits, and a 26 percent increase in breast cancer risk is too high a price to pay, even if there were a heart benefit," said Dr. Claude Lenfant, director of the National Heart, Lung, and Blood Institute, which ran the study.

Erectile dysfunction (ED) has been reported by 12% of men under 59, 22% of men aged 60 to 69, and 30% of men 70 and over. Being overweight makes it even more likely. Men with a body mass index (BMI) over 28.7 have a 30% higher risk of having ED than those with a normal BMI (i.e. 25 or below). Physicians in Naples, Italy, have studied whether weight loss and increased physical activity can improve ED in overweight men.

Obese men with erectile dysfunction, aged 35 to 45 years, were recruited. They had to have a body mass index of 30 or above, and a score of 21 or less on the International Index of Erectile Function (IIEF). Men with diabetes, high blood pressure, or raised cholesterol levels were excluded from the study.

Of 140 men assessed initially, 30 were eliminated - for health reasons or because they were unwilling to undertake the suggested lifestyle changes. The remaining men were allocated at random to join either the so-called 'intervention' group, or the 'control' group.

Men in the intervention group received detailed advice on how to reduce their body weight by 10% or more - individually tailored dietary recommendations, and guidance on increasing physical activity. The trial lasted 2 years, during which the men had regular sessions with the nutritionist and the exercise trainer - monthly in the first year and bi-monthly in the second.etc...
