General Steps
- Purchase the required number of boxes of ACUVUE Brand Contact Lenses within 90 days of your eye exam. See Display above for specific requirements for this ACUVUE product.
- To receive a Rebate form visit www.acuvue.com/rebates and download the form.
- Fill out the rebate form and follow its instructions.
- Mail by January 31, 2008 to:

Purchase must be made between January 1 and December 31, 2007, and received at the above address by January 31, 2008. Product Purchase must be made within 90 days after eye exam. Limit one rebate per customer, per offer, per ACUVUE® brand purchase, per yearly eye exam visit. This offer not valid in combination with any other offer or rebate. Photocopy of the certificate is not valid for redemption. Allow 4-6 weeks for delivery. No P.O. Boxes, only street and rural addresses are acceptable.
