To update your address or phone number please FAX us a letter with your 7-digit provider number and your NPI number with your old information and new information; or call:

To Update your Professional license you may FAX a copy of your license to 360-725-2144. Make sure you include your 7-digit Medicaid provider number on the professional license.
If there has been a change in Ownership or your TAX ID Number has changed you must resubmit a complete provider application with all supporting documents.
