UKSDMC SELF DECLARATION MEDICAL SYSTEM
UKSDMC SELF DECLARATION MEDICAL FORM - Download PDF file.
This is the standard form which members use to declare that they have no underlying medical condition that would potentially prevent them from diving safely. It should be noted that this form does not have any tick boxes on it because this form is intended to be retained by the Diving Officer and so should not contain confidential medical information.

REFEREES EXAMINATION FORM AND CERTIFICATE
Should a member be required to visit a Referee for examination because of having to answer 'Yes' to one of the questions on the self-declaration form, they should obtain a copy of the Referee's Examination form. This has the same questions but this time with tick boxes and a section for additional examination criteria. The candidate should complete section required and then take the form for their examination. If the examination is completed successfully the Referee will complete and sign the Medical certificate and a copy of this should be given to the Diving Officer.

UK SPORT DIVER MEDICAL FORM B
To be used should a member wish to continue to have a medical signed with a doctor, or the Branch continue to require such a medical in addition to the self-declaration process or one is required for diving at an Overseas location which requires this then the 'old' Medical form is still available for this purpose and is now known as Form B.

DIABETES & DIVING
Diabetes does not necessarily prevent a member from taking up diving, or for existing members continuing to dive if they contract the condition.
The UKSDMC have devised a specific process for diabetics to follow to determine their suitability for diving. There is a designated Medical Referee that deals with these enquiries and the 'Diabetes Pack' can be downloaded here:
