Why Talk About It?

What would happen:

  • if you were faced with a serious illness?
  • if an accident left you unable to communicate?

Would your loved ones know what your wishes for medical care would be?

Most of us have not prepared for this.  We may not even have thought about it.

What can result is:

  • confusion and disagreement among family members
  • unnecessary pain
  • inappropriate treatment with great discomfort
  • failure to consider comfort care or hospice care

Thinking about, talking about, and doing advance care planning can help you to assure your final wishes. Do it for your loved ones and yourself.

Better Ending provides three documents to help you
with advance care planning:

Guide for a Better Ending, an eight-page guide to assuring your final wishes.

Personal Wishes, a one-page form for recording your wishes under various circumstances.

Massachusetts Health Care Proxy Form, a two-page form, conforming to Massachusetts law that designates your Health Care Agent and Alternate.
