Did you know if an unwed, adult couple has a newborn and wants to add the father’s name on the baby’s birth certificate, they can complete a Voluntary Acknowledgment of Paternity form (VPA). The VPA form can be obtained at FDL Co CSA and we can assist in notarizing your signatures. Parties then mail the completed form to Vital Records in Madison, Wisconsin with the appropriate fee. The first certified copy of the birth certificate is $20.00, and ANY ADDITIONAL certified copies for only $3.00 each!!
If either parent wants genetic testing, the parents should not sign the form until they receive the test results. Parents may ask the Child Support Agency for genetic tests before signing the Voluntary Paternity Acknowledgment Form. Click here to find out more about the VPA.
