Contractors Prequalification
Contractors that want to work as a prime contractor on INDOT projects or LPA projects let through INDOT, must be prequalified with INDOT. Subcontractors can enter into subcontracts up to $300,000 without being prequalified with INDOT; however, the subcontractor is limited to a total of $300,000 in unearned work under contract on any given date with any owner, contractor, or other party.
INDOT only accepts original, hard copies of the contractor prequalification application. A fillable PDF of the application is below for your use.
Prequalification Application
In July 2016, INDOT revised the prequalification application, Contractor’s Statement of Experience and Financial Condition. The new application features substantial changes from previous versions. After January 1, 2017, INDOT will only accept the 2016 version of the application. If you have any questions regarding the changes to the application, please contact K.D. Thurman at 317-232-5095.
Contractors Statement of Experience and Financial Condition (2016 prequalification application)
New applicants should also submit these items in addition to the prequalification application:
- W-9, which includes the company name, address, and FEIN. An officer or owner of the company must sign and date it.
- Domestic corporations, limited liability corporations or limited partnerships should attach a current copy of the company’s Certificate of Existence from the Indiana Secretary of State
- Résumés of key employees
