NEW LICENSURE APPLICANTS PLEASE READ!
If you plan to apply for your Kentucky credential in the last two months of 2016, please be aware that Kentucky has a fixed deadline for licensure renewal. The next renewal deadline is March 31, 2017, and all physical therapists who have been issued a license prior to January 1, 2017, as well as all physical therapist assistants who have been issued a certificate prior to January 1, 2017 will be required to renew in order to continue to practice in Kentucky. THERE WILL BE NO EXCEPTIONS.
If you will not be working in Kentucky as a physical therapist or physical therapist assistant before January of 2017, you have the option to withhold your application until after January 1, 2017, in order to avoid paying the renewal fee. Alternately, you have the option to submit your application now and request in writing that your credential not be issued until after January 1, 2017.
Please print and complete the CATCH 22 form and mail it with your licensure application and fee in order to alert us to your preference. If no preference is indicated, then your license/certificate will be issued as soon as all requirements for issuing the credential are met.
The 2017 Jurisprudence Exam is available. This exam is required for all new applicants, reinstatement applicants, and current credential holders for the licensure biennium of 4/1/2015 through 3/31/2017.
Effective 6/24/2015, HIV/AIDS courses are no longer required for initial licensure or renewal.
