Note: If submitting a form to DMV, please print it on plain white paper, and note that some printers may require use of the "Shrink to Fit" printer dialog box to be checked in order to print forms on a single page.

Using DMV Forms & Publications
Fillable Forms
Fillable forms allow real-time electronic data entry and printing via a browser. The updateable areas of a fillable PDF form may be completed and then printed on your local printer. Once printed, the completed form may be submitted via mail or in person to DMV. Completed forms may not be submitted electronically (as with electronic forms), and data input into a fillable PDF form via a browser can not be saved using your browser's save functions (must use the Adobe Acrobat Reader instead).

When entering data into a fillable form via a browser:
  • Mouse-click in an updateable text box and a blinking cursor will appear.
  • Press Tab to go to the next updateable field.
  • Press Shift+Tab to return to previous updateable field.
  • To insert a checkmark into a checkbox, use the spacebar or mouse-click in the box.
  • Before printing, tab out of the field in which data was last entered.
  • To print, click on File and select Print (make sure the checkbox "Print as Image" is checked).
  • All DMV fillable forms have a CLEAR FORM button that removes all of the data you have entered and resets the form.

Standard (non-fillable) Forms, Manuals & Other Publications
Standard (non-fillable) forms and other publications allow downloading and local printing only.
You can not enter data into a standard (non-fillable) form, as it is merely a digital image of the form. Once printed, the form may be filled out manually and then submitted via mail or in person to DMV. Standard (non-fillable) forms can not be submitted electronically to DMV (as with online forms).

Audio Manuals & Other Publications
Audio versions of manuals and other publications allow downloading and playing of a recorded reading of the material.

Online Forms
Online forms allow real-time electronic data entry and submittal to DMV. The system generally prompts for input and then allows the user to submit the form to DMV for processing.

General Information
Please print, complete and submit ALL pages of DMV forms. If the form requires a signature, DMV must receive the form with an original signature.
 
Downloading & Printing DMV Forms & Publications
Fillable Forms
You may complete fillable DMV forms with your browser and print them on your local printer using your browser's Print option. With most browsers you may save a copy of the blank form by using the Save As option. However, data entered into fillable PDF forms can not be saved via your browser's save options (Save or Save As) due to limitations with the underlying technology (must use the Adobe Acrobat Reader instead).

Standard (non-fillable) Forms, Manuals & Other Publications
You may print standard (non-fillable) DMV forms and other publications on your local printer using your browser's Print option, or save a copy on your computing device using the Save As option.

Brochures are generally provided in a two-sided, tri-fold format. This means that they are designed to be printed on both sides of a single page and then folded in thirds. This requires duplex printing capabilities, or each side may be printed on a separate page and then converted by a photocopier with 1 to 2 side conversion capabilities.

Online Forms
Some online forms provide a local print option. Also, in most cases, the Print Screen option on your computing device may also be used. Online forms may not be copied to your computing device.

General Information
Some printers may require use of the "Shrink to Fit" printer dialog box to be checked in order to print on a single page. Some printers may require the selection of 1 or 2 sided output if they have duplex printing capabilities. Please print forms on plain white paper only (do not use colored, textured or patterned paper).
 
Portable Document Format (PDF)
Oregon DMV provides forms and other publications in Portable Document Format (PDF), which is a file format created by Adobe for document exchange. The PDF format represents documents in a manner independent from a specific application software, hardware, and operating system. Each PDF file encapsulates a complete description of the document that includes the text, fonts, images, and graphics. Version 6.0 or better of Adobe Acrobat Reader is required to view and/or print DMV files. You may download the Adobe Reader from the link provided in the PDF File Accessibility section at the bottom of your screen.

Fillable PDF Forms
DMV offers selected PDF forms that are updateable or "fillable." You can enter data into updateable fields within these forms using your browser, print them on your own printer, sign them, and mail or take them to DMV. The data entered into these forms is for printing only, as they can not be saved via a browser (must use Adobe Acrobat Reader instead). Refer to the sections above for more information about fillable forms.
