Person-Centered Planning
A Person-Centered Plan (PCP) focuses on the strengths, interests and needs of an individual. PCP is an ongoing process designed to help consumers choose the methods and resources and find their own pathways to success. The planners help the consumers figure out where the consumers want to go and decide the best way to get there. This leads to greater inclusion as members of both community and society.
Documentation and Training
Three hours of PCP documentation training is required for anyone who is responsible for writing PCPs. It is not required to use the curriculum developed by the Division of Mental Health, Developmental Disabilities and Substance Abuse Services (DMH/DD/SAS), however, it is offered as an option to complete the training requirement. Everyone must have records of the 3 hours of training, regardless of where it was obtained.
CABHA Person-Centered Thinking/Recovery Training Information
- Completion of the 12 hour Person-Centered Thinking training by a trainer certified through the Learning Community for Person Centered Practices
- Completion of the original 6 hour Person-Centered Thinking training requirement and the additional 6 hour MH/SA Person-Centered Thinking/Recovery training.

Provider Monitoring
The Provider Monitoring process is designed for:
- Entry into the provider network
- The evaluation of service providers against quantitative and qualitative measures
- For determining advanced placement status
The provider monitoring process is used to monitor both Medicaid and State-funded behavioral health services.
Review Information and Tools
Routine monitoring consists of a routine review and a post-payment review.
- For Licensed Independent Professionals (LIPs) that includes the LIP Review Tool and the LIP Post-Payment Review Tool.
- For provider agencies, the selection of tools is determined by the type services provided.
- The Routine Monitoring Tool is used except when the agency provides Unlicensed AFL services, in which case the Unlicensed AFL Review Tool is used.
- The specific post-payment review tool(s) to be used is based on the combination of services included in the sample.
- For those services that DHSR-MHL surveys on an annual basis (i.e., residential services and opioid treatment services), only a post-payment review is done.

NC SNAP Examiner Certification Training
NC SNAP Examiner Certification Training is only available to people who are required to complete or review NC SNAP assessments as a part of their job responsibilities.
NC SNAP assessments can only be administered by certified NC SNAP examiners.
If a person receiving developmental disability (DD) services does not have an assigned Care Coordinator, a Qualified Professional (QP) will administer NC SNAP. The local management entity/managed care organization (LME/MCO) is responsible for assigning the administration of the NC SNAP to a DD service provider.
We recommend that the following individuals receive training:
- Case manager Supervisor
- Care Coordinator Supervisor
- Case Managers
- Care Coordinators
- Qualified Professionals assigned to administer the NC SNAP
To register for NC SNAP examiner certification training, you must be referred by, and register through, your LME/MCO.
Once you have completed the training, you will be assigned a certification number. You must record your number on all completed NC SNAP assessments.
