Q6: My five-year Professional Educator License expires June 30th. I have not completed the professional development requirements.  I am not planning on teaching full time next year but would like to hold a five-year substitute license.

In order to locate the 5-year substitute license application (T001-6094 application), you first need to know the license number of your most recent teaching license. To do this, find your license record on our online license lookup, found here, https://elo.wieducatorlicensing.org/datamart/publicSearchMenu.do.

If you know your DPI entity (file) number, you can search by entity number, or if you do not know your entity number you can search by name:

Locate your most recent teaching license (please note this means a regular teaching license, not a previous substitute license).  Note the last 4 digits of the License number:

On your Quick Start Menu in ELO, look for the header that says “Manage Your Existing License(s).”  Under this section, you will see the licenses that you have held.  Look for the license number that matches the license number you found in License Lookup

There will be a drop down menu next to this license.  One of the options will be “Uncommon Request 6094.”  Select this, and then click “GO!”
