To get started, please download and fill complete these two intake forms. Please Print them and bring them to your first appointment with your biblical counselor. (If you do not have a printer, no worries. We have extra at our offices. Please come 15 minutes early in order to complete them.)

1. The Consent form gives you information about scheduling procedures, payment, policies etc.
2. The PDI asks you to complete information such as family background, health information, religious background and your goals for counseling. If you don’t feel comfortable answering any part of this form, leave that portion blank and you can discuss it privately with your counselor. There are two identical forms below (you only need one), the first is a Microsoft Word Document that you can type into and the second is a printable PDF version that you hand write.
