Proof of Claim Form and Instructions
There are three ways a Proof of Claim may be filed with the court:
- by mailing or delivering a paper copy to the Clerk's Office in Phoenix, Tucson or Yuma;
- by using our on-line Electonic Proof of Claim (ePOC) application;
- or by facsimile (fax).
The instructions below provide guidance for online filing and fax filing.
Instructions for Filing an Electronic Proof of Claim (ePOC)
This claim filing process is not for use by attorneys or by creditors who have filed more than 10 claims in this court. Attorneys and creditors who have filed more than 10 claims in this court must file claims via the CM/ECF system. It is also not available if you are filing a claim pursuant to FRBP 3002.1 (claims secured by a security interest in a chapter 13 debtor's principal residence).
A proof of claim may be filed online using the electronic proof of claim (ePOC) application. If you are filing a proof of claim electronically for the first time, please read all information provided below BEFORE attempting to file your proof of claim. This service is provided as an alternative filing option for those parties not eligible to register for CM/ECF who previously had to file their proofs of claim conventionally (on paper) with the court.
A claim filed electronically on this court’s website will constitute the filer’s approved signature and have the same force and effect as if the filer signed a paper copy of the document. Documents required to be verified or contain an unsworn declaration that are filed electronically will be treated, for all purposes (both civil and criminal including penalties for perjury), the same as if signed or subscribed.
Certain personal information must be removed from all documents before electronically submitting the proof of claim form. All filers must redact: Social Security or taxpayer-identification numbers; dates of birth; names of minor children; and financial account numbers, in compliance with Bankruptcy Rule 9037. This requirement applies to all documents including attachments.
You will be prompted to upload attachments before submitting the claim. Attachments uploaded via this web site MUST be in a PDF format, MUST be smaller than 7mb in size (larger documents can be broken down into multiple documents that are smaller), and MUST NOT contain document security.

Instructions for Filing a Proof of Claim by Fax
This claim filing process is not for use by attorneys or creditors who have filed more than 10 claims in this court. Attorneys and creditors who have filed more than 10 claims in this court must file claims via the CM/ECF system.
You may fax file a Proof of Claim to the court if you are a creditor (the person who is owed money in a bankruptcy case) and are not represented by an attorney (form below).
Please complete the fillable form and fax it to the court at 602-682-4004. You should also include additional attachments supporting your claim such as invoices. Certain personal information must be removed from all documents before faxing the proof of claim form. All filers must redact: Social Security or taxpayer-identification numbers; dates of birth; names of minor children; and financial account numbers, in compliance with Bankruptcy Rule 9037. This requirement applies to all documents including attachments.
The faxed claim will be held until reviewed by a court employee and, if accepted, will be considered filed as of the received date of the fax. A Proof of Claim cannot be accepted for filing if you do not reference the bankruptcy case number on the form. If you have received a pre-printed Proof of Claim form from the Court, please use it as it displays the debtor’s name as well as the case number.
