MyDownstateCare is your secure, online health connection, because your health is important to you around the clock – not just during business hours. That's why Downstate offers the MyDownstateCare Patient Portal. With this Internet connection to your medical records you can view your hospital discharge instructions any time!

MyDownstateCare is available exclusively to inpatients, emergency room patients, and ambulatory surgery patients of SUNY Downstate Medical Center. We will soon be adding more features – keep checking back for updates on what information you can access using the MyDownstateCare patient portal.

MyDownstateCare

What is MyDownstateCare?

MyDownstateCare Patient Portal is a secure online home for your inpatient hospital health information. It enables you to securely use the Internet to help manage and receive information about your health. With MyDownstateCare, you can conveniently use the Internet to view your demographic information and discharge summary. We will be adding helpful new features regularly, so check the portal frequently.

There is no cost to sign-up or use MyDownstateCare. This is a service Downstate is providing to help you become a healthier, more informed patient.

It's easy to sign up.

Here are a few simple steps to follow while you are at the hospital.

  • Present your photo identification when you register, and provide your email address.
  • You will be given written instructions when you register as an inpatient, and within 24 hours you will be sent an invitational email with a link to the MyDownstateCare web page.
  • Select the link, go the patient portal, and follow the instructions you received when you registered to set up your account, including the “invite code.”
  • Your invite code is created by using the last 2 digits of your birth year, followed by the last 2 digits of your phone number. For example: If your birthday is May 21, 1990, and your phone number is 123-4566-5678, your invite code is 9078.
  • After entering the invite code, follow the onscreen instructions to create a username and password of your own choosing. Once your MyDownstateCare account is created, for all future log-ins, you will simply need your chosen username and password to log in.
  • Need help? See the Technical Questions sections in this FAQ, or call the patient portal hotline at 718-270-6372 between the hours of 9:00 am to 5:00 pm. After hours, you can send an email to MyDownstateCare@downstate.edu.

It's easy to use.

You don't have to download or install any programs. You can access MyDownstateCare from any desktop computer, tablet, or smart phone with internet connection. MyDownstateCare is user-friendly and easy to navigate. All you have to do is set up your account and activate it by logging in.

It's available to most of our inpatients.

If you are 18 years of age or older, you can establish your own account.

Parents and guardians of children ages 0-10 years of age may establish what is called “proxy access” to a child’s account. However, under state and federal guidelines, this type of access expires once a child turns 11 years old.

Children 11 to 17 years of age cannot establish an account on MyDownstateCare on their own or through a parent/guardian at this time.

Your Medical Record

When can I see my health information?

Your demographic information and discharge summary will be posted in your account within 36 hours after you leave the hospital.

If there are concerns with some of the information on MyDownstateCare, what should I do?

Your MyDownstateCare information comes directly from your Downstate electronic medical record.

As with all medical records, if there are discrepancies or if there are questions regarding your patient record, contact, visit, or call the SUNY Downstate Medical Center Health Information Management Department to request an amendment of your medical record, at 1-718-220-2494.

"Proxy Access": Can I view a family member's health record in MyDownstateCare?

Yes, if you are granted what is called "proxy access" to the patient's account.

Parents/ Legal Guardians – Parents or legal guardians of children ages 0- 10 years of age may request proxy access. Legal guardians must present guardianship or other legal documents that authorize access to a child's health information before proxy access can be established. Under state and federal law, proxy access expires when a child turns 11 years old.

Dependent Adults – Adults with a designated personal representative who has authority to make healthcare decisions for the patient must present appropriate documentation to establish proxy access to that adult patient's account. For more information, call 1-718-270-6372.

Family members – You can allow a family member to have proxy access to your account by following the instructions in the MyDownstateCare portal. SUNY Downstate Medical Center can't create this type of proxy access for you, but we will be happy to provide technical advice on how to do it. Call 1-718-270-6372.

How do I terminate proxy access?

If you gave proxy access to a family member or other adult, you can deactivate that access at any time, directly from your MyDownstateCare account.

Can my spouse and I share one MyDownstateCare account?

No, due to the sensitive nature of health information, each adult must complete a separate consent form and establish a unique MyDownstateCare account.


Technical Questions – Signing Up

What equipment do I need to sign up?

You can use any computer with internet access or a smart phone with Internet Explorer 8.0 or higher, or modern versions of Chrome, Firefox or Safari. If you are using your smart phone –iPhone or Android – you must first install a mobile app called "FollowMyHealth."

Why did my email invitation for the portal come from "FollowMyHealth"?

SUNY Downstate has partnered with Allscripts Healthcare to provide our patients with the opportunity to participate in a patient portal.

"FollowMyHealth," abbreviated "FMH," is the name of the portal supplied by Allscripts.

Because MyDownstateCare is brought to you by Allscripts, you will see "FollowMyHealth" on all email correspondence from the portal. You will see "FMH" on the secure login page and you will see "FMH Universal Health Record" on the terms of use. These abbreviations and names all refer to the patient portal.

How do I sign up?

  1. You will be asked to sign a consent form and provide your email address to office staff when you register.
  2. The Admissions staff will verify your information and help you develop a personal invite code.
  3. Once your information is verified, you will receive an email inviting you to sign up.
  4. In this invitation, a link will be provided that will take you to the MyDownstateCare login page.
  5. Because this page is supplied by Allscripts Healthcare (see above), you will see a button that says FMH, for FollowMyHealth. Do not be concerned. This is the correct page to log in to MyDownstateCare.
  6. After entering the invite code, follow the onscreen instructions to create a username and password of your own choosing. Once your MyDownstateCare account is created, for all future log-ins, you will simply need your chosen username and password to log in.

What if I did not receive an email after I signed up?

If you don't see your invite in your inbox, please check your spam, junk or quarantine file. If it's not there, call the MyDownstateCare hotline at 718-270-6372 for assistance or email MyDownstateCare@downstate.edu

When I was trying to create my username in FollowMyHealth (FMH), I got a pop up message saying "Username already exists." How can this be and what do I do?

When creating your username, you may get an alert that tells you "Username already exists. Please enter a different username."

Because FMH lets you create any name you choose for your account, it is possible that someone else who already has a FMH account has chosen that same name for their account. The system will not let two users have the same username. Make a change to the username you selected (like adding a number or extra letter to the beginning or end) and try again. Please make sure it is something you will remember.

Technical Questions – Logging On

What do I need in order to access MyDownstateCare?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

How do I log on for the first time?

To Register:

  1. Check your email. Click the registration link in the email you receive from noreply@followmyhealth.com." ("Follow myHealth) is the company Downstate's contracts with to provide MyDownstateCare. )
  2. Follow the onscreen prompts, starting with the "Create an Account" button.
  3. On the second screen, click on the "FMH" secure log-in button.
  4. On the third screen, enter a unique username and password. Click "Continue."
  5. On the fourth screen, re-enter the username and password you just created. Then hit the log-in button.

To Connect:

Follow the onscreen prompts to Connect Your Account.

  1. Accept the Terms of Service.
  2. Enter your invite code – the last 2 numbers of your date of birth, followed by the last 2 numbers of your phone number. (For example: If your birthday is May 21, 1990, and your phone number is 123-4566-5678, your Invite Code is 9078).
  3. Click "Agree" to the Release of Information.

Your health record will begin to upload. This may take a few minutes.

My invite code does not work. What should I do?

For your security, your invite code expires after 60 days and is no longer valid after the first time you use it. If you still have problems, email us at MyDownstateCare@downstate.edu or call us at 718-270-6372.

I was logged out of my MyDownstateCare, what happened?

We aim to protect your privacy and the security of your information. While logged into MyDownstateCare, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out. We recommend that you log out of your MyDownstateCare session if you need to leave your computer for even a short period of time.

What if I need help logging in after I leave the hospital?

Call us at 718-270-6372 between the hours of 9:00 am to 5:00 pm. We'll be happy to help. For after-hours assistance, email us at MyDownstateCare@downstate.edu.

How secure is the MyDownstateCare Patient Portal?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure invite codes, as well as personal ID's and passwords. Each person controls their password, and the account cannot be accessed without that password.

MyDownstateCare uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyDownstateCare. Unlike conventional e-mail, all MyDownstateCare messaging is done while you are securely logged on to our website.

What is the Privacy Policy for MyDownstateCare?

Your privacy is important to us. The information you provide on the portal is protected by federal laws. Please see the Privacy Policy in the terms and conditions section of the FollowMyHealth web site.

Technical Questions – After I Have Enrolled

If I forget my password, what should I do?

You can click the "Forgot Password" link on the sign-in page to reset your password online. If you encounter any problems email us at MyDownstateCare@downstate.edu or call us at 718-270-6372.

Can you send me a new invite code as I have lost it, let it expire, or did not receive it?

If you have already logged into MyDownstateCare and have created a password, then you must reset your password. (See above for instructions.)

If you have not yet created your MyDownstateCare account your invite code will be the last 2 digits of your birth year followed by the last 2 digits of your phone number.

For example: If your birthday is May 21, 1990 and your phone number is 123-456-5678. Your pass code is: 9078.

How can I update personal information (such as home address or email) or change my password)?

Log into MyDownstateCare and from the left menu, go to the Preferences section and select the appropriate option.
