HL7 HQMF eMeasureIdentifies if the measure is an “eMeasure.” To be considered an “eMeasure,” a clinical quality measure must be encoded in the HL7-approved Draft Standard for Trial Use (DSTU), Health Quality Measures Format (HQMF). The HQMF standard was developed to enable specification of measures to achieve the goal of repurposing existing clinical data. Measure NumeratorThe upper part of the fraction that enumerates the cases of interest from the denominator’s population of interest.Text Field

Measure DenominatorThe lower part of the fraction that enumerates the total population of interest, from which the cases in the numerator arise.Text Field

  • Number of patients 18 years of age and older (with at least one visit during the reporting period) who were discharged alive for acute myocardial infarction (AMI), coronary artery bypass graft (CABG), or percutaneous transluminal coronary angioplasty (PTCA) (during January 1 to November 1 of the year prior to the measurement year), or who had a diagnosis of ischemic vascular disease (IVD) during the current or prior year.

Donabedian Measure Type Classification

1) Outcome
These are measures of “a change in the patient’s current and future health status that can be attributed to antecedent health care.” These include measures of, “undiagnosed disease, preventable disease, morbidity, mortality, disability…and improvement of social and psychological function…as well as measures of “patient attitudes (including satisfaction), health-related knowledge acquired by the patient, and health-related behavioral change.” Also includes measures of practitioner outcomes, such as measures of practitioner satisfaction.

2) Process
These are measures of the “activities that go on within and between practitioners and patients.” It includes measures of clinical care, but also measures of timeliness of care, utilization, and measures of the manner in which practitioners deal with patients; e.g., measures of concern, courtesy, respect, maintaining patient privacy and confidentiality and providing enough time for the patient.

3) Structure
“The concept of structure includes the human, physical, and financial resources that are needed to provide [health] care.” Structural measures assess “the relatively stable characteristics of providers of care,…the tools and resources they have at their disposal, and…the physical and organizational settings in which they work...” These include, for example, measures of the characteristics of the setting and health system in which healthcare takes place; the number, distribution, qualifications and turnover of personnel; the number, size, equipment, and geographic disposition of health care facilities; and how the healthcare system is arranged such as having provisions for coordination of care, information management, drop-in care, emergency care, coverage at night and on weekends, and home visits.
