Customized Living and Foster Care Services

Lead agencies and providers may refer to the CBSM Rate methodologies for AC, ECS and EW service authorization page and the Long-term services and supports rates changes page for additional information on determining rates and authorizing services for the Elderly Waiver (EW),Elderly Waiver Residential services, Alternative Care, Essential Community Supports programs, and Foster Care services.

Effective July 1, 2015, lead agencies are required to use the Residential Services Workbook (RS Tool), formerly known as the CL Tool, to determine rates for EW foster care and residential care services
Choose the service for which you are determining a rate from the drop down box in the ‘Authorize Rate As’ field on the ‘Scr Doc Input’ worksheet in the RS Tool. The tool will allow lead agencies to plan for component services that are allowed under each service.

Tools
The name of the tool has changed from “Customized Living Tool” to “Residential Services Tool.” As of July 1, 2015, lead agencies are required to use the tool for service planning and rate-setting for adult foster care, in addition to customized living services.

Please regularly check for and download updated tools. DHS will update these tools periodically.

Responsibilities
- All customized living, adult foster care, and residential care providers must enroll in Minnesota Health Care Programs. Applicable forms, instructions and technical assistance are located at DHS Provider Enrollment.
- County or tribal lead agency staff act as agents for the state Medicaid agency (DHS) and are responsible to administer programs.
- Counties, tribes and health plans using county networks or using DHS’ EW Residential Services Tools need to submit them to DHS within 15 days of the service authorization customized living line item begin date.
