Levaquin/Cipro

  • Brand Levaquin and Cipro will only be authorized if the recipient meets Brand DAW criteria.**

Factive/Avelox

  • Clinician must document clinical failure or intolerance to levofloxacin before authorization of Factive or Avelox OR
  • Cultures show sensitivity to gatifloxacin and moxifloxacin but  not levofloxacin for respiratory tract infections

Background information

In the majority of clinical cases levofloxacin will treat suspected fluroquinolone susceptible respiratory infections and are therefore the preferred agents.
