Resolving problems if you are not in a health plan

People who are not in a health plan get their health care through fee-for-service. Fee-for-service means that you can get care from any health care provider who accepts Medical Assistance (MA).

File an appeal with the state
If the Minnesota Department of Human Services makes a decision about payment or authorization of a particular service and you disagree with the decision, you can appeal. You must appeal in writing. You can use the form below or write a letter including all the information that the form requires.
