The US women qualified for Tokyo by winning the 2019 Parapan American Games title. They are looking for their third Paralympic gold medal at Tokyo 2020, and their seventh medal in total. Canada are the only other team to have won two Paralympic titles, and no other side has finished on the podium more times than the United States of America.

At Rio 2016 they defeated Canada 2-0 in the last eight, before losing 11-1 in the semifinals to a Turkish team that went on to win gold. The US team then triumphed 3-2 in a closely contested bronze medal match against hosts Brazil. In Tokyo they are looking to claim gold for the first time since the 2008 Games in Beijing, when they defeated the Chinese team in the final. They also won gold at the inaugural Paralympic women's goalball tournament in 1984.

At the 2018 World Championships the US women were eliminated by Turkey in the quarterfinals. It meant they did not secure a medal for the first time in five world championship appearances since 2002. In that period they won two world titles, including in 2014 when they overcame the Russian Federation in the final.
